  2. What is Passport? • Helps people who have a developmental disability who have left school or have been waiting for service and are in need of community participation supports

  3. What is Passport? • This part of the initiative can give you and your family a creative, unique way to meet your needs • It is meant for adults who are seeking community participation supports and no longer in school • Voluntary & based on a commitment from your family & a willingness from you to take part in these supports • If you choose the option of direct funding, you can: • Purchase supports from existing community agencies or • Hire your own worker Funding for Community Participation Supports

  4. What is Passport? • Funding for community participation supports under Passport is a limited resource - not an entitlement program • Depending on available resources, some individuals may or may not receive funding Funding for Community Participation Supports

  5. Who can apply for Passport funding? • Live in Ontario (Canadian citizen, landed immigrant, Minister’s permit, permission to remain by Citizenship and Immigration Canada, e.g. refugee) • Developmental disability • A student age16 or 17 may apply for the Passport Program while they are still in school, but the application cannot be funded until the individual has turned 18. • Are an adult living in the community who has been waiting for community supports and due to needs require this support • Are not eligible to receive support through the Ontario Disability Support Program (ODSP) Employment Supports Program (You can be receiving an income from ODSP and apply for Passport funding) .

What can the funding be used for? • Passport applications will only be considered on an individual basis (no group submissions); however, if you are approved for Passport funding, you and your family can pool your resources with other families to set up a program for your group • Community participation supports’ = a support person or service that helps you: • Prepare for a job • Volunteer in the community • Learn how to get along with others, live on your own or look after yourself • Continue your education after high school • Learn how to shop for yourself, use the library, the bus, find out what’s at your local community centre, etc.

  7. What can the funding be used for? Option #1 Buy community participation supports and services Example: Hire your own support person to help you learn a job Option #2 Use your funding to access services through community agencies Example: Use staff from an agency that provides supports and services to help you Option #3 Combination of Option 1 & 2

What can the funding be used for? • Up to 10% of total Passport money can be used to: • Hire a person of your choice, including an agency, to look after paying your service provider (e.g. writing a cheque to your service provider, keeping track of your receipts for you and sending your receipts to your local delivery agent • Have someone help develop & coordinate a plan for the community participation supports

How do I know if Passport is right for me? • Passport funding = choice about how to get community participation supports • More choice about how you get and spend your support money = more responsibility • If you choose direct funding option, need to make sure you understand your responsibilities • Here are some things to think about…

Things to think about… • If you are approved for funding & you are in a day support program now – you need to decide between using the Passport funding or staying in the day support program – you can’t do both. If you choose Passport funding, you need to give up your day support program BUT do not give up your day support program until you hear if you have been approved for Passport funding • Applying for Passport funding can give you more choice about how you get your supports but it does not mean that you will get the same amount of support you were getting in your existing agency based service

Things to think about… • If you choose to hire a worker, you are now the employer – as an employer, you will need to find the right worker, interview the worker, ask for and check references, schedule hours, etc. or find someone to help you with this • Workers you hire are self-employed – they are not employees of the Ministry of Community Social Services or employees of the community agency

Things to think about… • It is your responsibility to make sure the program you select provides a quality service and is the right program for you • You should have an agreement – something written down on paper - with the service you use or the individual you hire so that everyone is clear about what your needs are & what service the person will be giving you An agreement can help you sort things out if there are problems or disagreements If you would like to see sample agreements, go online and visit respiteservices.com)

  13. Things to think about… • You or your family member (or you may find someone to help you) are responsible to manage the money that you get from Passport. This means: • You have a responsibility to use the money only for what was approved in your plan • You pay the service provider directly and you need to work with the service provider to sort out any problems • You need to send in receipts to York Support Services Network There are many things to think about but the direct funding option to purchase your own supports or purchase supports through an agency may be just the right way to meet your community participation support needs.

How do I apply for Passport funding? • Developmental Services Ontario can be contacted at 1-855-277-2121 • Developmental Services Ontario will confirm eligibility under the Services and Supports to Promote the Social Inclusion of Persons with a Developmental Disabilities Act, 2008 (SIPDDA), and if eligible, complete the Developmental Services Application Package • The Developmental Services Ontario organization will refer eligible Passport Program applicants to the local Passport Designated Agency that administers the Passport Program and provide the agency with the information collected from the Developmental Services Application Package Contact Developmental Services Ontario

How do I apply for Passport funding? • To the extent possible, individuals should apply for the Passport Program with a plan that outlines the type of community participation supports they require and the goals and objectives they expect to accomplish • A plan will help you and other people understand what you want to do, what you need, how you want to do things and who you want to help you • A plan can also tell you and other people what community participation supports you need Complete an Individual Plan

How is my application processed? • The Passport Coordinator will: • Review your application package • Determine a funding level Step #1: Application reviewed Remember, when you apply for Passport funding, you may be eligible but that doesn’t automatically mean you will get funding. Many people will have community participation needs and the government wants to support people based on their needs but there are limits on how much money it has to meet these needs.

  17. How is my application processed? • The information from all applications submitted are reviewed for prioritization. Consideration is given to your needs, other people’s needs in your community, and how much Passport funding is available in order to make allocation decisions. • The Passport Coordinator will then let you and your family know in writing if you have been approved for funding and if/when you can begin accessing Passport supports. Step #2: Priority Review

  18. What if I move? • If you have been approved for Passport funding and decide to move to another community in Ontario, the same level of funding is available to you whether you choose to hire your own support worker or purchase supports through an agency • If you decide to move, you need to take some steps to make sure your funding follows you to your new community: • Contact your local delivery agency within a reasonable amount of time before you move to let them know when you are moving and your new address • Make arrangements in your new location for community participation supports. You are responsible to make sure the quality of service and support you need in your new community meets your needs. • Make sure you understand how funds will be flowed to you by the delivery agency in your new location
