An Easy Process • All applications must have a Personal Health Interview done prior to submission • The interview and other medical information results will guide the coverage entitlement. • Agent will be notified of situations where coverage entitlement is more or less than applied for and allow 24 hours to advise us not to proceed. • Choice of certificate effective date • Checks received with applications will be cashed and “held” until the certificate effective date. • Preferred pre-authorized checking date is available (the 29th, 30th, and 31st are not available)
