Course Objective
  • Gain a thorough understanding usage of the Universal Client GUI and the Consumer Preferences Profile GUI.
  • These have been installed and maintained at the central IHS headquarters location.

IHS NHIE Overview - Goals
  • Enable IHS Cross-Facility health data exchange as well as health data exchange with external organizations over the Nationwide Health Information Network (NHIN)
  • Enable each IHS Facility to maintain ownership and control over their patient data

  • Open Source Software gateway that facilitates Health Information Exchange over the Nationwide Health Information Network
  • Implements Core Services to Product Specifications laid out by the Nationwide Health Information Network Initiative

  • A few examples of the core services are
    • Document Query - to obtain patient document references
    • Document Retrieve– to obtain patient document
    • Patient Discovery – to correlate enterprise patient identities across the Nationwide Health Information Network
  • Every Agency/Private Health Institution that uses CONNECT/equivalent gateway is termed a NHIE

CONNECT GUIs
  • CONNECT includes two GUIs as a part of its distribution
  • Both have been customized for IHS usage
  • The two GUIs are
    • Universal Client GUI- to view patient documents from IHS’ NHIE and from across the Nationwide Health Information Network
    • Consumer Preferences Profile GUI– to manage patient consent at IHS’ NHIE

  • Used by Healthcare Providers
  • Facilitates viewing of Patient Documents within IHS’ HIE and/or the Nationwide Health information Network
  • A typical workflow entails
    • Login to the GUI
    • Search for a Patient
    • Query for Documents
    • Retrieve Patient Document
    • Patient Discovery (Optional)

  • Provider logs in with D1 Credentials
  • Username format is “D1\<username>”
  • Provider clicks “Login” button

Universal Client GUI –Patient Search
  • Provider enters search criteria, Patient Demographics, and clicks “Search” button
  • First and Last name are required criteria
  • Search results are probabilistic – Best match is the first entry in the table
  • Provider selects required patient from table

Probabilistic Matching
  • Patient search using Demographics is probabilistic and not exact
  • Weights are assigned to different search criteria. Ex: First Name, Last Name, DOB etc. MPI uses this and determines best possible match, in addition to variants.
  • Pros: Useful during search for a patient if incomplete search information is provided.
  • Cons: Patient Search could return exact match and much more (Ex: Next-Of-Kin). The best match is the first record returned.

  • Only required criteria is the “Creation Date Range”, defaulted to one year.
  • “Search IHS HIE using HIEOS XCA” check box is checked by default – searches only IHS HIE
  • Provider clicks “Document Query”

  • Document Style-sheet can be selected to render document content, default – “IHS Standard Template”
  • Provider selects required Document from the table

  • Not a part of a typical workflow
  • Preliminary step to be performed if subsequent “Document Queries” need to access the NHIN.
  • Provider clicks “Discovery Patient” on the NHIN
  • Table is populated with correlated enterprise patient ids from the NHIN

Consumer Preferences Profile GUI
  • Used by Administrators
  • Facilitates establishment of Patient Consent at the IHS HIE
  • Patient consent determines if Patient Documents are shared with External Agencies or Private Healthcare institutions on the Nationwide Health Information Network
  • Patient Documents will always be shared for “Treatment” and “Emergency” purposes. Consent affects other purposes of use such as “Public Health”
  • IHS has adopted the “Opt-In with “Purpose-of-use” model. Patient is “Opted-out” by default.
