Getting Started All full-time employees must complete a health/welfare benefits enrollment form and provide proof of dependency • Required health & welfare benefit elections • Medical and dental plans – to enroll or waive • Flexible spending accounts – to enroll • Optional benefit elections • Supplemental life insurance and Short-term disability insurance • Vision Insurance • Other voluntary benefits (MetPay, Hyatt Legal, pet insurance, etc.) – via provider Web site Your enrollment form must be submitted to the Benefits Department within 31 days of your hire date Your Pegasus health and welfare benefit elections will be effective on the first of the month following your date of hire You will automatically be enrolled in the Pegasus 401(k) plan unless you opt out of the plan within your first 31 days of employment. Your Pegasus 401(k) plan and health and welfare elections/deductions will begin the first paycheck of the month following 31 days of full time employment

Getting Started When enrolling for the first time or making a qualified change during the year or during annual enrollment you will need to submit appropriate documentation to support proof of dependency. • Definition of Eligible Dependent: • A lawful spouse • Your unmarried children who • Are less than 26 years of age; • Over 26 years old and primarily supported by you and incapable of self-sustaining employment by reason of mental or physical handicap.

Getting Started • Acceptable Forms of Proof: • Spouse • Marriage certificate • Affidavit of common law marriage (in states where legal) • Dependent Child • Certificate of live birth • Certificate of adoption • Medical support order (for court ordered coverage) • Certificate of legal guardianship • Previous year’s tax records showing your name, your spouse’s name, and the name of your dependents (if applicable), and the signatures of both you and your spouse (financial information can be blacked out)

Group Medical Benefits Two Group Medical Plans To Choose From… • Core Medical Plan (you assume more responsibility for minor medical costs and enjoy lower payroll contributions) • Buy-up Medical Plan (plan assumes more responsibility for minor medical costs and you assume higher payroll contributions) “Aetna Open Access Managed Choice POS Network” • Direct access to care – any provider can be used, but out-of-network costs may be higher • No referrals needed for specialists • No claim forms – in-network claims filed by provider • Preventative visits paid at 100% • Ask Aetna questions directly at 888-416-2277 or www.aetna.com. You may view provider networks online. Once you receive your ID card you may register in the site to view your benefits.

Dental Plan Benefits • Deductible • $50 Individual • $150 Family • Covered Expenses • Preventative care – 100% • Basic restorative – 80% (after deductible) • Major restorative – 50% (after deductible) • $2,000 annual dental maximum per participant • Orthodontia • 50% (after deductible) • $1,500 lifetime maximum per participant • Dependent children only

  16. Life and AD&D Ins. Benefits • Basic Life / Accidental Death & Dismemberment Insurance(company paid) • 2X salary up to $500,000 (minimum coverage = $15,000)* • Optional Life Insurance (employee paid)* • Employee • 1X, 2X, or 3X salary up to $1 million • EOI required for benefit amounts in excess of 2X salary to $300,000 • Spouse (employee must elect at least 1X salary in optional) • Up to 100% of employee supplemental life benefit up to $250,000 • EOI required for any amount over $50,000 • Children (must elect at least 1X salary in optional) • $5,000 or $10,000 • $0.40 or $0.80 per month • EOI not required for initial enrollments • * The benefit reduces 33% at age 65 and 50% at age 70. Refer to Plan Document for details.

  17. Short & Long-term Disability Ins. Benefits • Short-term Disability (employee paid) • 14-day elimination period • 60% of salary up to $1,000 per week (minimum= $25/week) • Benefit paid for 24 weeks (168 days) after elimination period • Seamless transition from short to long-term disability • After-tax premiums = tax-free benefit • Long-term Disability (company paid) • 60% of salary up to $11,000 per month (minimum= $100/mo.) • 180-day elimination period • Benefits received are taxable

Flexible Spending Accounts Health Care Spending Account • Pre-tax contributions for expenses not covered by medical or dental plans Deductibles, co-insurance, and co-pays • Over the counter medications can no longer be reimbursed through FSA’s unless you provide Adviant with a valid physician’s prescription • Employees can choose pre-tax contributions of $100 to $2,500 for plan year 2014 • Immediately available (not a pay-as-you-go benefit) • Services MUST be INCURRED AND PAID prior to reimbursement. You cannot be reimbursed for a PREPAID health care expense. Orthodontia is the exception to this rule. Dependent Care Spending Account • Pre-tax contributions for eligible dependent care expenses • Employees can choose pre-tax contributions of $100 to $5,000 per plan year • Pay-as-you-go benefit

Flexible Spending Accounts You contribute money to your account on a pre-tax basis to pay for eligible health care or dependent care expenses • Advantages • Less taxable income • Lower income tax • Lower Social Security tax • More take-home pay • Submit receipts for reimbursement by mail, fax, or scan/email – reimbursement will be through payroll • Pay with debit card Payroll reimbursements for allowed health care and dependent care expenses are not included in your W-2 income, nor are they subject to taxes and withholding. Federal tax law allows these reimbursements to receive favorable tax treatment for dependent children and dependent relatives who meet the IRS definition of a “dependent” for tax purposes.

Flexible Spending Accounts • Convenient Visa debit card that enables Pegasus Solutions FSA participants, and their spouse, to instantly pay for FSA qualified healthcare and dependent care expenses with pre-tax dollars wherever the card is accepted • No need to file paper claims and wait to be reimbursed through payroll if debit card is used at a compliant merchant • Purchases at non-compliant merchants may require you to provide receipts and back-up documentation to the plan administrator (but not at point-of-sale) • FSA debit card account access available online • FSA participants will receive 2 cards Detailed FSA debit card information can be found at www.pegshr.com Once you receive your BennyCard you may register on line at www.mybenny.com Member ID: your SSN Card Number: your BennyCard number Zip Code: zip code on record

Flexible Spending Accounts • Forfeitures • Use it or lose it – Federal tax law requires that you forfeit any amounts not used in your account after all claims related to that plan year have been submitted • You have up to three months after the end of the plan year (March 31) to submit claims for expenses incurred during the previous plan year (Jan 1 to Dec 31) • Forfeited money will be used to defray administrative costs of the Plan

Pre-Paid Legal (Hyatt Legal) • Plan gives you and your family access to legal services • Attorney fees fully paid by the plan when you use an in-network attorney • All enrollments are for the remainder of the calendar year – no changes can be made during the year • No limits on the number of times you may use the plan, and no dollar limits on use of an in-network plan attorney for the following services: • • Purchase, sale or refinancing of your primary residence • • Trusts, wills/codicils, living wills and powers of attorney • • Document preparation • • Immigration assistance • • Identity theft and debt collection defense • • Traffic ticket defense (no DUI) • • Elder law matters • • Uncontested adoption or guardianship • • Defense of civil lawsuits • • Eviction and tenant problems (primary residence tenant only) $13.33 per pay

  26. 401(k) Retirement SavingsAuto Enrollment • You will be automatically enrolled in the Pegasus 401(k) plan • A deferral equaling 2% of your eligible compensation will be automatically contributed to the plan • Auto-deferrals will be directed into the plan’s qualified default investment, unless you make a fund change • Manning & Napier Pro-Blend Extnd Term S– a moderately aggressive allocation fund;  a balanced asset mix which strikes a middle-of-the-road path between stocks and fixed-income investments • Auto-deferrals begin the first payroll check of the month after 31 days of full time employment unless you… • Negatively enroll yourself (“opt out” of the plan), or • Make an alternate election of 1% – 25% of your eligible earnings

  27. 401(k) Retirement Savings Example: Eloise Employee Date of Hire: July 13th Interim Dates: 31 days of employment satisfied: August 13th 1st of month following 31 days employment: September 1st First 401(k) Payroll Deduction taken: September 15th The first payroll check Eloise will receive following 31 days of continuous employment will be her September 15th check.

  30. 401(k) Retirement Savings - PostAuto Enrollment • Change your deferral % at any time, the change becomes effective at the beginning of the next calendar quarter • Change your deferral % to 0 at any time, within the quarter – may not re-enter the plan until the beginning of the following quarter • Contribute 1 – 25% of W-2 pay up to the IRS limit of $17,500 for 2014 • IRS limit on catch-up contributions is $5,500 (attained age 50, or will attain age 50 during the 2014 plan year, and over) • Pegasus will match up to 3% of contribution • All enrollments/changes are made online at www.millimanbenefits.comno later than2:59 PM CDT on the last business day of each quarter in order to be effective at the beginning of the next quarter
