Support Services • Providers, did you know that the Your Healthcare Plus Program™ can assist Disease Management consumers by: • Providing appointment reminders • Assisting with transportation for provider appointments • 24 hour nurse triage support (800) 973-6792 • Locating a Medical Doctor • Assisting with problem-solving to remove barriers to access services • Extending provider support between appointments by telephonic and home based outreach • Supporting adherence to your care plan as prescribed • For more details on these support services, please contact • felicia.vertin@mckesson.com

Primary Care Providers and Mental Health/ Substance Abuse Providers Collaboration With few exceptions, Primary Care Providers (PCPs) have been assigned to all Your Healthcare Plus eligible members/consumers through Illinois Health Connect. It is helpful when community mental health and substance abuse providers: • Obtain consent to communicate with the PCP at the start of services • Obtain contact information for the consumer’s PCP • Contact the PCP preferably in the consumer’s presence or with their permission • Coordinate the mental health, substance abuse and/or medical care needs with the assigned PCP For more information or questions, please contact felicia.vertin@mckesson.com

Coordination is Critical Recently, the Substance Abuse and Mental Health Services Administration (SAMSHA) provided an excellent teleconference titled “Addressing The Challenges Of Medical Providers In Treating Persons With Mental Health Issues.” It was reported that the life span of individuals with major mental illness is shortened on average by 25 to 30 years when compared to the population at large. Co-morbidity and increased barriers to accessing healthcare were key factors. This is a compelling reason why it is so important for consumers to access physical health care services. Encouraging consumers to see their PCP will assist in all our efforts to extend the lives of the consumers we serve.

  6. Proud Practices Community Counseling Centers of Chicago - Clients Express Themselves The annual consumers’ holiday art exhibition show is near! Community Counseling Centers of Chicago (C4), give clients a unique way to express their feelings and thoughts through the creative process of art therapy, one of many treatment modalities used at all C4 sites throughout Chicago. The skillful facilitation of therapeutic art expression and public exhibitions of client artworks demonstrates the strengths, resilience and creative talents of the consumers served. The art is also displayed in the main reception areas and has been exhibited in galleries, coffee shops, and other public venues throughout the city. This is a way to increase public awareness of mental illness and recovery. Some art works are available for sale in the lobbies of each site and will soon be available to purchase online through C4’s website. Proceeds of sales are given to the artist. C 4’s art therapy services have been in existence since 1985 and were developed by Joanne Ramseyer, LCPC, A.T.R., and Coordinator of Expressive Services. There are three additional full-time registered, licensed art therapists on staff, as well as graduate level interns who provide the services to children, teens, adults and bilingual clients. No art experience or talent is necessary to participate in group, family or individual art therapy sessions. We applaud C4 for providing this service to consumers.   7. Flu and Pneumonia ShotsIntegrated Services It’s that time of the year! Flu and pneumonia shots preventively promote better health and have been known to save lives. The role of health providers in promoting the physical well being of consumers is front and center. Community Mental Health and Substance Abuse providers can support the consumer by encouraging preventive care. Your Healthcare Plus staff can help you help the consumer get preventive shots this Fall.   8. Say It Out Loud Individuals desiring or needing mental health services are often reluctant to access care. The "Say It Out Loud" campaign provides key education tools and encouragement beneficial for individuals who are, or are aware of others, who may be struggling with the realization of good mental health as vital to their general wellbeing. "Say It Out Loud" is an exciting, strength-based teaching intervention that facilitates acceptance, especially for newer consumers. Hail to the Troops Providers with Disease Management consumers that have spouses, parents, or children serving in the Military full time away from their home base, are asked to contact fawzia.shuyab@mckesson.com Our team of nurses would like to ensure that these consumers know about all the community resources available to Military families.

  10. Collaboration Success Story • The consumer began participating in the Your Healthcare Plus program in June 2007 (after we located her in the hospital). This high cost high risk member has schizophrenia, depression, asthma, and hypertension. The consumer had a history of frequent multiple hospitalizations for mental health issues, suicide attempts, medication non-compliance, lack of medical and mental health follow-up in primary care setting, and lack of transportation. The consumer also stated that she was not getting along with her parents with whom she resided. Our Behavioral Health Specialist assisted the consumer with obtaining a medical home as well as a psychiatrist and a therapist. Our Social Worker assisted the consumer with transportation, food, and provided her with housing resources. • In October 2008, our Social Worker conducted a wellness check/home visit. The consumer reported she has been meeting with her psychiatrist monthly, her therapist weekly, and her primary care provider (PCP) when needed. The consumer also reported that she feels better then she ever has, her relationship with her family has improved, and she continues to live with her parents. Food and transportation are no longer an issue for her and are no longer preventing her from attending to her medical and mental health needs. Provider Support is Key During a recent office visit with a consumer, I had the pleasure of engaging a provider who not only was aware of the Your Healthcare Plus program, but verbalized positive feedback about what Illinois Healthcare and Family Services is trying to accomplish. This provider actually turned to a section of the consumer’s chart and exhibited letters and updates he received from Your Healthcare Plus. He indicated that he read them prior to putting them into the chart. This provider shared his concerns about the reasons people are still utilizing the Emergency Room instead of making provider appointments. One problem he sees is transportation challenges. Another problem he mentioned is that consumers are not being seen often enough in the office. He stated that he has started having his consumers come in monthly if they have been known to frequent the Emergency Room, or if they have been suspected of abusing medication. He feels that this has improved the healthcare outcomes and reduced Emergency Room visits. This was one of the most positive provider encounters I have had and I wanted to share it!
