Providers for state funded services shall demonstrate a Continuous Quality Improvement (CQI) process by identifying a minimum of3 Improvement Projectsacted upon per year.
Quality Improvement Projects (QIPs) test change in the real work setting by:
1. Project Title – Give it a name
2. Timelines – Start and end dates – if project is in process provide the projected completion date
3. Who will be affected by the project. Consumers or staff?
4. Who and Why – Name the staff responsible for the project and why they were chosen
Identify - what specific target do you want to improve?
Why did you select this project? Did data or information indicate there was a problem?
