Quality Improvement Projects (QIPs)

  • WHY?

    Providers for state funded services shall demonstrate a Continuous Quality Improvement (CQI) process by identifying a minimum of3 Improvement Projectsacted upon per year.

Quality Improvement Projects (QIPs) test change in the real work setting by:

  • Analyzing the problem

  • Evaluating alternatives

  • Testing/implementing solutions

  • Acting on what is learned

  • Standardizing to ensure useful actions are preserved


What goes in a qip
What goes in a QIP

1. Project Title – Give it a name

2. Timelines – Start and end dates – if project is in process provide the projected completion date

3. Who will be affected by the project. Consumers or staff?

4. Who and Why – Name the staff responsible for the project and why they were chosen

Identify - what specific target do you want to improve?

Why did you select this project? Did data or information indicate there was a problem?

Reasons for selection:

  • Connected to an agency strategic goal

  • Identified Risk Management Area

  • Data agency collected indicated a problem

  • Identified Gap


Identify your data and begin to analyze
IDENTIFY YOUR DATA AND BEGIN TO ANALYZE

  • Identify your base line data – What numbers are you looking at? Admissions, staff turn-over rate, incidents, etc.

  • Look at relationships between numbers- example: Jan-5 incidents, Feb-8 incidents, March-35 incidents – What happened in March??

  • You can link your QIP to your strategic goals, example: Your agency goal is to become a leader in substance abuse training for adolescents. Your QIP is to train clinicians on 7 Challenges.

  • ANALYZE THE PROBLEM – WHAT IS CAUSING IT?

  • Is it due to lack of knowledge or experience?

    Consider an educational program

  • Is the problem an inefficient or ineffective work process?

    Staff Training and Supervision

    Technology Improvements

  • Identify actions to reduce or eliminate the problem

  • Select the action you want to take

  • Decide what data you need to take to see if your action is working

  • Set up a standard form for documenting your agency’s QIPs

  • Use cross-functional/interdisciplinary teams to bring different perspectives and skills

  • Include other stake holders

  • Try a “blitz team” (a short term team that commits significant time to the project)

  • Clearly define the data are you collecting to see if the project is working.

  • How often will you collect it? At what intervals?

  • How are you collecting it?

  • What is your source of the data?

  • Define/explain any instruments or tools you are using to collect data.

  • How do you know if the data is reliable?

  • Are you using a billing warehouse? They can provide you with data.

  • Excel

  • Use what you already have available

  • Think about what you currently are required to do and use that data.

  • What you are measuring should tell if you are making projected improvement – What is it telling you?

  • Use concrete measureable indicators

  • How are you measuring?

  • Are you measuring what you need to?

  • Be specific and objective

  • Was the project a success?

  • If it improved, was it the result of planned intervention? (This is face validity )

  • If not, what went wrong? It is OK if it didn’t work – there is benefit in the process.

  • What are the next steps to take? Identify the barriers to success. Document your steps to overcome those barriers.

  • How are you going to maintain the improvement?

  • When will you review the data again?

    • Monthly, Quarterly, Annually?

    • Who will look at the data?

  • Create a standing agenda item related to Quality Management in staff meetings.
