Continuità assistenziale tra ospedale e territorioPerché? • Gli Ospedali sono destinati ad occuparsi sempre di più dei pazienti acuti. • Gli ambulatori specialistici e dello SCC non possono farsi carico della gestione di tutti i pazienti con SCC. • Il miglioramento delle cure dello SCC passa attraverso un progetto integrato a livello territoriale (distretto) in stretta collaborazione tra le diverse figure coinvolte.
