  2. Background • Funded by the Ontario Ministry of Health and long term care & Sault College • 2 year grant • Free service to those who are experiencing barriers to participate in physical activity • Project Leader

  3. Our goal • Our ultimate goal is to increase physical activity levels and healthy eating habits of individuals who are experiencing barriers. Our health outcome is to improve their quality of life and prevent chronic disease.

  4. Target Population • The project aims to engage at least 200 people per year (400 total over 2 years) in diverse programs. • Cancer, Persons with physical or intellectual disabilities, SCI patients, diabetes, obesity, youth, deaf and hard of hearing, osteoporosis, native, blind, and low income moms and babies

  5. Program Structure • No Cost • Use of space at Sault College • Student Placements for various programs • Pre and post program fitness assessments • Emphasis on specialized physical activity programming for the target population and • Delivery of healthy eating and general health education

  6. Our Programs • Cancer Survivors • First Steps • Deaf Fit • SCI Fit • Healthy Kids and Families Obesity Program • Moms on the Move/Strollerfit • Youth

  7. Cancer Survivors • First program established – 2008 • Model for development of the CHLP • Average of 12 participants every class (total of 50 overall) • Results

  8. First Steps • Targeted to those with physical or cognitive disabilities • Average of 25 participants

  9. Deaf Fit • Deaf and Hard of Hearing • Average of 25-30 participants • Results – 100% lost an average of 9 lbs. • Future

  11. Moms on the Move/Strollerfit • Moms and Babies – most successful program to date. • 60 participants • Results

  15. Healthy Kids and Families • Obesity Program • Partnership with Dr. Keuhl • Worked one on one with 4 clients • Donations from the GHC, Goodlife, Diabetes Association, and Essar. • Program Expansion

  16. SCI Fit • Those with a spinal cord injury (paraplegic and tetraplegia) • Use of the new physical activity guidelines from SCI action

  17. Future Projects • Osteoporosis • Diabetes& Obesity Collaboration • Blind Fit • Prostate Cancer Survivors Class for Men

Sustainability • Close to November, we will be starting to look for additional funding to keep the programs running. • The program has been very successful in that we are getting people moving in the community of Sault Ste. Marie, who were experiencing barriers to participate before. • Our goal is to make Sault Ste. Marie THE most active, healthy and inclusive community.
