  3. HUMAN RESOURCES OFFICE • Contact the Human Resources Office to request forms, contract information or for questions on employee benefits. Forms are also available on the college’s website at: • www.nwcc.commnet.edu/humanresources/forms.htm

  4. IMPORTANT FORMS • Forms: • ** Federal W-4 Form: Change the amount of Federal Income Taxes withheld from your bi-weekly paycheck • ** State W-4 Form: Change the amount of State Income taxes withheld from your bi-weekly paycheck • ** Direct Deposit Form: Have your bi-weekly paycheck automatically deposited into your bank account • ** Retirement Form: Changes to your Beneficiaries and address • Employee Data Form: Changes to your name, address, telephone numbers, emergency contacts (Contact Information for NCCC files) • ** These forms can ONLY be changed at your PRIMARY Agency.

DUAL EMPLOYMENT If you work for another state agency, including teaching at another community college, or UConn, or one of the CSU’s, you must complete the Dual Employment section of your contract. The first agency that hired you is considered your “PRIMARY” agency. Personnel records are maintained at the primary agency. Both agencies must approve your dual employment form.

BENEFITS – TUITION WAIVER • Part-time lecturers who are in the seniority pool at a college, and non-teaching unit members employed for more than ninety (90) scheduled working days and commencing with the third consecutive semester, shall be eligible for space available tuition waivers on the following terms. • Tuition and fees will be waived for one credit course per academic semester to be taken at the college where the unit member is in the seniority pool and where employed. • The waiver may be used by the employee, or his/her spouse or any of his/her dependents during a semester when the unit member is actually working, except that if a course or courses assigned to the part-time lecturer is cancelled after a space available tuition waiver as described herein has been granted, the waiver may be used during said semester.

BENEFITS – HEALTH INSURANCE • The Office of the Comptroller informed units of public higher education that effective with the Fall 2007 semester, certain adjunct faculty would be eligible for state-subsidized health insurance.   11. CODE OF ETHICS • The provisions of Code of Ethics Policy shall apply to all employees of the Connecticut Community Colleges. It shall be the responsibility of each employee to be familiar with these provisions and to comply with them. It is strongly suggested that employees avoid those situations which are ethically questionable or which may give the appearance of so being. FURLOUGH DAYS • The SEBAC * Agreement recently ratified by its members calls for the establishment of six (6) furlough days for fiscal years 2009/10 & 2010/11. • For FY 2010 NCCC will be closed on: • Friday, November 27, 2009 • Thursday, December 24, 2009 • * State Employees Bargaining Agent Coalition
