Intake and Output • Is a means of recording all fluids a person takes in and eliminates during a certain period of time. • Intake is all fluids that are put into the body. • Output is all fluids that are expelled from the body.
What is intake and output used for? • A balance between the amount of fluid taken into the body and the amount lost from the body. • Helps a physician provide care to the pt. • In healthy individuals, their fluid balance usually maintained by the body structures. • If pt has heart or kidney dz or loses large amounts of fluids from vomiting, diarrhea, excessive perspiration, or bleeding, fluid balance may be abnormal.
Types of Intake: • Oral • Intake of liquids through the mouth. • Nurse assistants measure and record these amounts.
Types of Intake • Tube Feedings (enteral feedings) • Used for pts who are unable to swallow. • These pts could be unconscious, in a comatose state, or have a digestive disease. • Types of Tubes • Nasogastric- through the nose • Gastrostomy- surgically inserted through the abdominal skin to the stomach

Types of Intake • Intravenous(IV) • Fluids given into a vein. • Blood units, plasma, etc. • Measurements are the responsibility of the nurse.
Types of Output • Bowel Movement, BM • Liquid bowel movements are usually measured and recorded. • A solid BM is usually described under feces.
Types of Output • Emesis • Anything that is vomited is measured and recorded. • Color, type, and other facts are usually noted.
Types of Output • Urine • All urine voided and drained via a catheter is measured and recorded.
Irrigation • Forinput • Refers to fluid into tubes that have been inserted in the body. • For output • Any irrigation or suction drainage from nasogastric tubes, hemo-vacs, and other drainage tubes.
Measurements • All amounts must be measured in graduates. • A graduate isa container made of plastic and has calibrations for mL/cubic and/or ounces on the side.
