  2. Purpose • Employees can make any changes to their pre-tax insurances – Health, Life, FSA, Dental, Other Supplemental Coverage • Post-tax insurance (i.e., coverage through The Gabor Agency) or Retirement is not included. FGCU - Benefits Section

  4. People First • All enrollments/changes must be made through People First (Benefits Administrator). • All enrollments/changes must be made by 5:30pm on October 26th. • UserID and password required for changes (Service Center 866.663.4735) FGCU – Benefits

Confirm Changes • How do I know my changes went into effect? • People First confirmation statements • Instant online statements (web enrollment) • Home mailings (after Open Enrollment has ended) • Erroneous information – call People First immediately! • Deadline: Friday, November 9th by 5:30pm • Changes made during Open Enrollment will go in effect January 1, 2008. • Review FGCU paycheck (Dec. 7th) Changes? Health Insurance • Same Premiums Available: • Standard: $50/mth Individual, $180/mth Family • Health Investor: $15/mth Individual, $64.30/mth Family • Same Providers Available: • PPO - Blue Cross/Blue Shield • HMO - AvMed or United Healthcare • TriCare Supplement will no longer be available after December 31, 2007! • Must select option from PPO, HMO or no provider • If no change made, employees will default to PPO FGCU – Benefits

  8. Reimbursement Accounts /Health Savings Accounts • Please review current election! • Must select annual amount when enrolling (deductions based on number of paychecks) • FSA – “Use it or lose it” • Used with the Standard Health Plans • HSA – “Rolls over” • Used with the Health Investor Health Plan • New employee contribution maximums • $2900 (individual) and $5800 (family) • No change in employer contribution maximums • Employee catch-up provision of $900 (age 55+) FGCU – Benefits

  9. Life Insurance • New vendor: Minnesota Life • Policy: Group Term, incl. Accidental Death & Dismemberment • Plan: Core benefits unchanged • Enhancements: Will preparation & Beneficiary financial counseling • Value: Basic, 1.5x annual salary Optional, 1-5x annual salary • Beneficiary: Must submit new beneficiary designation form to provider (available through People First) • Default to spouse, child, parents, or estate FGCU – Benefits

Vision Insurance • No vision insurance available during the 2008 Plan Year • Current coverage will end 12/31/2007 • Deductions will end after 11/21/07 paycheck • Suggestion: Enroll/increase Flexible Spending Accounts for eye exams, glasses and/or contact lenses • Vision screenings available as preventive benefit under PPO and HMO providers (through co-pay) and discounts given on glasses and contacts • Check with carrier for specific rates and products FGCU – Benefits

Dental Insurance • Nine Available Options: • Pre-paid (DHMO) • American Dental Plan, Assurant (Denticare), Cigna, CompBenefits & United Healthcare (Starmark) • Dental PPO (DPPO) • CompBenefits • Indemnity • Ameritas, Assurant (Denticare), CompBenefits • Coverage through Oral Health Services (DHMO & DPPO) will end 12/31/07 • You will default to CompBenefits or United unless you make an active election(default selection made by People First) • You will default to new DPPO plan under CompBenefits unless you make an active election. Dental Cont. • Pre-paid/DHMO • Low out-of-pocket costs, limited provider freedom (network only) • DPPO • Higher out-of-pocket costs, more provider freedom (in-network vs. out-of-network) • Indemnity • Higher out-of-pocket costs, complete provider freedom (no network) • Pay for services up front, file for reimbursement FGCU – Benefits

Dental Cont. • New tier system: • Employee (EE) • EE + Spouse • EE + Child(ren) • Family • Employees currently enrolled in any coverage level (other than employee only) and no change is made, coverage will default to the family tier level regardless of the number of dependents. Supplemental Insurance • Hospitalization, Cancer/Intensive Care, Accident/Disability, Optional Life • If enrolling, please complete additional paperwork or meet with a company representative. • Enrollment not complete until People First/Insurance Company approves underwriting. FGCU – Benefits

  16. Suppl.   17. Suppl. Insurance Cont. • Alta is offering an addition to the 365 Plus HMO supplemental hospitalization plan • PPO suppl. plans available: 30/20 or Preferred Provider Plus • Choose hospitalization coverage to help pay for out-of-pocket expenses not covered by health insurance • Hospital deductible, room & board, co-pays, etc. • To enroll, complete application (found in brochure) and enroll through People First • Vision discount offered (including eye exam, glasses and contacts) FGCU – Benefits
