  8. Reduce High-Risk Pressure Ulcers Goal 1:Nursing home residents receive appropriate care to prevent and minimize pressure ulcers • National average 12.8% • Wisconsin state average: 10.7%

  9. Reduce Physical Restraints Goal 2: Nursing home residents are independent to the best of their ability and rarely experience daily physical restraints • National average 6.3% • Wisconsin state average: 2.3%

  10. Reduce Pain in Long-Term Residents Goal 3: Nursing home residents who live in a nursing home longer than 90 days infrequently experience moderate or severe pain • National average 5.1% • Wisconsin state average: 4.4%

  11. Reduce Pain in Post-Acute Residents Goal 4: People who come to nursing homes after staying in the hospital infrequently experience moderate to severe pain • National average 21.2% • Wisconsin state average: 26.7%

  12. Set Targets Goal 5: Setting individualized targets for clinical quality improvement (STAR Target Setting) • Wisconsin: Nearly 1/3 of NHs have set targets over the last two years

  13. Setting Targets, Cont • Research shows that nursing homes that regularly set QI targets are more likely to improve the quality of care they provide to their residents. • We hope that 90% of Wisconsin nursing homes will voluntarily set annual clinical quality targets on www.nhqi-star.org.

Resident and Family Satisfaction Surveys Goal 6: Nearly all nursing homes assess resident and family experience of care and incorporate this information into their quality improvement activities • National Goal:>80% of nursing homes regularly assess resident experience of care and incorporate into their quality improvement activities • Wisconsin: Unknown

Staff Turnover Goal 7: Most nursing homes measure staff turnover and develop action plans as appropriate to improve staff retention • National Goal: >80%of nursing homes regularly measure staff turnover & develop action plans to reduce the rate of turnover by 2008 • National average for staff turnover will be reduced by 15% • Wisconsin state average: 51% (CNA)

  16. Consistent Assignment Goal 8: To maximize quality as well as resident and staff relationships, the majority of nursing homes will employ “consistent assignment” • One-third of nursing homes will have adopted “consistent assignment” of CNAs to residents • Wisconsin: Unknown

  17. Consumer Involvement • The Board on Aging and Long Term Care encourages residents and families to get involved with this national campaign to improve the quality of life and care in nursing homes in the state of Wisconsin

Resident and Family Participation • Residents and families participating in the campaign will help create greater awareness of quality of care and the resources available now, and they will encourage providers to improve the care they deliver. • Residents and families should encourage facilities to focus their efforts on those goals that are most important to the residents.

  19. How to Participate as a Resident or Family Member • Go to campaign web-site at : www.nhqualitycampaign.org • Click on “For Consumers” • Read about opportunities for consumers to support the campaign • Fill out registration form • Begin communicating with others about the campaign

National Campaign Objectives Related to Resident and Family Participation • The campaign will: • Provide consumers the opportunity to participate in a national two-year campaign designed to improve nursing home quality. • Promote opportunities for resident, their families, ombudsman, citizen groups and other advocates to work with nursing facility staff on ways to achieve the campaign’s goals and to support facilities’ efforts to institute meaningful, lasting changes.

National Campaign Objectives - continued • Create opportunities for national, state, and local dialogues on nursing home quality that promote resident-directed care, culture change, improved quality of care and life for residents, and better working conditions for staff. • Provide consumers with concrete educational materials on the seven areas of quality being addressed in the campaign.
