Program Description • The post – rehabilitation Program „Second Step” offers therapeutic, educational, social activities aimed at strngthening therapy results and facilitating social readaptation of graduates of residential and out-patient therapeutic programs for adolescents and young people in Gdańsk district.

  3. The post-rehabilitation program “Next Step” offers residential care for people who are preparing to graduate from therapeutic program, as well as out-patient support for those who have graduated from therapeutic programs for addicted people and problem drug-users. • It is a continuation of the therapy program “Find Yourself” which is implemented by the MONAR Rehabilitation Centre, and the therapy program implemented by the Youth Therapy Centre in Sopot.

Program goals • relapse prevention • support in better social re-integration for graduates of therapy programs for adolescents and young people addicted to drugs • support to start independent living for residential programs graduates • additional therapeutic measures based on individual needs • provide conditions for successful life start and personal development of therapy graduates • prepare family/close community for return of the graduate • support for families participating in therapy of their child in rehabilitation centres.

  6. For Whom? • Hostel Residential program in Gdansk has 12 bed-places for people who graduate from therapy programs in residential centres of the Tri-city area. • - Out-patient program – support group and therapy for people who completed addiction treatment programs

Program Beneficiaries • clients of the MONAR Rehabilitation Centre for Adolescents and Young people in Gdansk, 2 Agrarna str. • clients of the MROWISKO Therapy Centre for Young People in Sopot, 817 a, Niepodleglosci str. • graduates of other therapy centres who are currently staying in the Tri-city area • families and friends of therapy programs’ graduates

  8. beneficiearies who are preparing to complete their therapy program or have recently graduated beneficiaries are of school education age (up to 21 y.o.) and normally continue school education in middle and upper schools in daily, evening or week-end formula. During their stay in the program, beneficiaries are required to maintain regular contact with the residential program and follow the individual care plan. Beneficiaries are obliged to abstain from drug, alcohol use, tobacco smoking and to follow the rules of the hostel. Average age 17-18 years old Description of the Program Participants

  9. Admission • Admission takes place 3 times a year. People who plan to take part in the post-rehabilitation program are required to apply formally providing justification to the manager of the program Next Step, • Admission criteria include: - assessment of implementation of individual care plan - concent of the group of beneficiaries and staff members - declaration to follow rules and program of the Program Next Step

  11. Hostel „Second Step” • Premises we operate is the building situated in the centre of Gdansk, the location favours implementation of the program goals, as well as beneficiaries’ active participation in cultural and social life of the city

Hostel • allows for independent living with possibilities of therapeutic and social support. • All program beneficiaries (12 persons) are at the similar therapy level and relatively in similar life circumstances. Their personal goals are similar. Size of the group is important too as it allows for intensive work and feeling of safety.

  16. Hostel • Therapeutic groups are aimed at personal growth, professional development, and increase self-esteem, strengthen ability to cope in stressfull or crisis circumstances. Better involvement, responsibility and independence of beneficiaries create special environment for such group therapy meetings.

  17. Hostel Program beneficiaries: • people preparing for graduation from therapy programs • people who have graduated from a therapy program but due to different reasons are to stay in the hostel for a certain period of time (3-months contract)

Ways of program implementation • provide settings for independent living for beneficiaries (food, equipment, educational materials etc.) • organise and participate in solving important tasks of beneficiaries; • organise workshops and training for development and education, involvement of specialists, providing promises, etc. • organise meetings with representatives of local communitites • provide access to specialised counseling based on personal needs of beneficiaries (lawyer, medical doctors, social worker, sexologist etc.) • organise cooperation with the job office, welfare institutions (also for participation as volunteers in different social projects for people in need) • cooperation with families and close community of beneficiaries • cooperation with schools

Methods of the program implementation • development and educational workshops aimed at preparing a beneficiary to take care of own tasks (health, social issues, work, study) and improving community awareness (rights and obligations of a citizen, local institutions, health care system etc.) • workshops aimed at developing interpersonal skills (to improve self-esteem, ability to establish and maintain contacts, friendship etc.) • workshops aimed at building capacity to cope with stress, crisis situations and to prevent relapse • supporting activities and therapy for beneficiaries and their families • individual counseling • specialised counseling • support group meetings for graduates of therapy programs of different residential and out-patient centres for addicted people and problem substance users

Out-patient Program • Support group activities for people who have graduated from therapy programs in different residential centres for prevention and treatment • admission takes place once a month. The group is of the open type. Admission criteria include: • abstinence from psychoactive substances • participation in several individual meetings with addiction therapist • declaration to follow rules and requirements of the post-rehabilitation program • Individual counseling and therapeutic support, help in solving important issues, counseling for families and friends of a beneficiary.

Expected Program Results • A) strengthen therapy results through • - strengthening commitment to abstinence • - preparing for independent life in society • - improving social commitment of young people • - increasing belief in own strength and social competencies • B) decrease risk of relapse among graduates of therapy programs (and reduce social harm) • C) strengthen belief of local community that a drug addicted person can re-integrate as a community member and a citizen. • D) provide equal chances for education, personal and social growth for people from disadvantaged social groups.

Measurable results (2004-2008) • 110 persons have completed therapeutic program of centres and conduct independent and drug free life • 90 persons have been employed (permanent or temporarily) • 30 persons have settled their criminal proceedings • 95 persons continue school education or vocational training • In the framework of the program “Next Step” an out-patient support group has been established for graduates of therapy programs and those who droped out of the program but are drug free; about 30 persons take part in this group meetings in a year scope. • About 130 persons of family members and friends of program beneficiaries have received therapeutic help through individual and group therapeutic meetings and counseling.
