  3. 二、期中轉介與適性鑑定 1.期中轉介: 每學期(上.下)都會有一次期中轉介 導師或任課老師,發現學校新個案,須要評估(對障礙類別的了解)ps.新個案:特教通報網沒有的名單就是

  4. 2.適性輔導安置鑑定 對象:九年級生,在特教通報網有名單之個案(全部) 目地:為升學或就業轉銜而準備 鑑定時間:上學期中(11/14.15)

  5. 流程 發現新個案 通報給特教組 評估(利用晤談及測驗工具) 確認後,蒐集學生之資料 取得家長同意書 知會導師 填寫鑑定表 心評老師(魏氏四) 再送鑑輔會鑑定,通過者,成為資源班服務對象(無論確認或疑似身分都是)

  7. 102學年度九年級升學之時程表 一.適性輔導安置鑑定會議(11/15)（已完成） 二.轉銜會議(102/12/24) （已完成） 三.報名(103.1/7-1/17) （已完成） 請家長先回去填 四.選填志願(12/16-12/25) （已完成） 邀請學生及家長一起 以上於今年度已完成！
