  2. HOUSEHOLD • Who’s included? • Individuals who live together in the same residence and who purchase and prepare food together • Must be US citizen or have acceptable alien status (DHS will evaluate alien status) • Make an entry for each household member

  3. UNEARNED INCOME • Fill in for each household member • Note: Unemployment compensation excludes $25 federal weekly benefit • Include also: Workers compensation, pension • NOT: educational income, DHS $50 “Child Support Pass-Through”, adoption subsidies • “Monthly” income = weekly income x 4.3

  4. EARNED INCOME • Fill in for each household member • NOT: work-study, EITC, SSI PASS income, Volunteer Stipends • If Self-employed: Enter Gross Earnings minus 25% (unless business expenses are > 25% of total proceeds) • Income must always be verified with DHS

  5. MEDICAL DEDUCTIONS • Enter medical deductions for each household member who is 60 or older, or receiving federal disability (SSI, SSD, Veterans Disability) • Does not have to be paid to be allowed

  6. DEPENDENT CARE EXPENSES • For children < age 18 or incapacitated adults • Must be necessary for household member to work, seek work, or receive training • Only unreimbursed expenses • Does NOT have to be paid to be allowed

  7. CHILD SUPPORT PAID • Enter amount a household member pays in child support • This can be current or for arrears • Must be court-ordered

  8. HOUSING EXPENSES • Enter amount paid for rent, mortgage, property tax, insurance, property assessment and other related fees • For all food stamp recipients, check box that says you pay heat separately. As of March 1, 2010, everyone is eligible for the standard heating/utility allowance.

  10. RESULTS • Compare household’s current benefit to projected amount • If client should be getting higher benefits, he/she should contact caseworker

How to Apply for Food Stamps • Pick up an application form (DHS 1171) from local DHS office, or call DHS to mail a form • On-line form is available at http://www.michigan.gov/dhs/ • Fax, mail, or drop off completed application to local DHS office • For help with filling out an application, contact DHS
