  2. What is PM&R? • What does PM&R stand for ? • Some think it stands for:PleasureMoney & Relaxation • However, it really stands for: Physical Medicine and Rehabilitation

  3. Another name for PM&R? • Physiatrist (fizz ee at’ trist) • Physiatry • Rehabilitation Medicine • Interventional Physiatry/Physiatrist • “Rehab” Doctor

  4. What is Physical Medicine • No it isn’t a physical therapist. • It is the use of physical agents in the care of patients, such as: • ultrasound • mechanical assist devices • electrical stimulation • manual medicine • therapeutic exercise

What is Rehabilitation? • The development or restoration of a person to their fullest physical, psychological, social, vocational and educational potential consistent with his or her physiological or anatomic impairment and environmental limitations. • In other words….. “putting the pieces back together “ !!

  6. How did PM&R originate? • The roots of PM&R date back to the use of physical agents such as the sun’s heat and hot springs for medicinal purposes in early recorded history. • The field began in earnest in the 1930s.

  7. More on the origin……. • After WW II, as thousands of veterans came home with serious disabilities, the task of helping to restore them to productive lives became a new focus for the field. • The Advisory Board of Medical Specialties granted PM&R its approval as a specialty of medicine in 1947.

  8. What is the goal of PM&R? • Work to obtainOPTIMAL FUNCTION and improved QUALITY OF LIFE !!! • Emphasizes the prevention, diagnosis, and treatment of disorders involving the neuromusculoskeletal, cardiovascular, and pulmonary systems

  9. Sampling of Diagnoses • Acute and Chronic Pain • Stroke • Spinal Cord Injury • Orthopedic joint replacement • Traumatic Brain Injury • Amputations • Sports-Related Injuries • Industrial Injuries • Performing Arts injuries

  10. Types of Practice • Outpatient - solo or within a specialty group • Interventional Pain • Non-operative musculoskeletal • Sports Medicine • Industrial/Occupational Medicine • Performing Arts • EMG/NCV consultant

  11. Types of Practice • Inpatient • Free Standing Rehabilitation Hospital • Transitional Care Units • Acute Care Rehab - usually a rehab floor within a large med center

What is the role of the PM&R physician ? • Work as the leader of aninterdisciplinaryteamto provide a comprehensive rehabilitation program for the patient • Provide expert opinion and services for referring physicians • Optimize the outcomes of surgical interventions through rehabilitation efforts

  13. Interdisciplinary Team • neuropsychologists • physical therapists • occupational therapists • speech and language pathologists • social workers • nurses • dietitians • recreational therapists • Orthotics/prosthetics specialist

  14. What is the future of PM&R? • As the population ages, as people survive conditions that once would have been fatal, and as quality of life is an increasing concern, the field of PM&R is moving to the forefront of medicine • PM&R workforce study

  15. PM&R Residency at UAMS • Four-year Program • 1 year of fundamental clinical skills • 3 years of PM&R training • Three residents accepted per year • Written Boards at end of residency • Oral Boards after first year of practice

  16. Fellowship TrainingCurrent there are no UAMS PM&R fellowships • Most PM&R Fellowships are one year in length • Sports 45.9% • EMG 13.1% • SCI 10.6% • Pediatrics 9.1% • TBI 7.3% • Stroke 3.6% • Pain 2.7% • Amputee 2.1% • Other 5.3%

  17. Didactics • Tuesday morning lecture • Seminar/Journal Club/Board Review on Thursdays • Grand Rounds 3rd Thursday of each month • Guest Lecture series - 2 to 3 times/year • Annual functional anatomy & physiatric diagnosis course

  19. Call Schedule • In-House Call at BHRI - PGY 2 - PGY-4 • Call on average is every tenth night with good sleep most nights • PGY-1 call on VA Wards, ACH and Baptist Rotations
