Corporate Job Bank – Workplace Safety Training Back / Lifting Injuries Common Causes of Back Injuries: • Reaching & lifting off balance • Improper training • Unaware of proper body mechanics • Lifting objects with awkward or odd shapes—too big • Lifting in uncomfortable positions • Slips—Trips—Falls • Attempting to lift or carry too many items • Twisting torso while lifting Preventing Back Injuries: • Avoid lifting & bending whenever you can • Place objects up off the floor • Raise and/or lower shelves • Use carts and dollies • Use lift-assist devices whenever possible • Test the weight of an object before lifting by picking up a corner • Get help if it’s too heavy or if too awkward for you to lift object alone • Turn/Rotate your feet, not your waist • (i.e. - don’t twist torso while lifting)

Corporate Job Bank – Workplace Safety Training Slips, Trips & Falls Causal Factors: • Hazards in work area • wet floors - water, ice, insufficient lighting, spills or chemicals not wiped up, etc. • Uneven walkways • Objects on floors • paper, paper clips, pens, equipment, debris, tools, boxes, cords, etc. • Inattention • reading cell phone/pdawhile walking down stairs • carrying items that obstruct view • Poor or insufficient lighting • Rushed Preventing Fall Injuries: • Maintain good housekeeping • Clean up tools, equipment or other debris • Report spills immediately • Pay attention to where you are walking • Maintain clear view of walking path • Use hand rails on stairs • Use lift-assist devices: dollies, carts, equipment transport devices • Properly store cords, equipment & supplies

  4. Corporate Job Bank – Workplace Safety Training RESTRICTED TASKS As an employee of Corporate Job Bank there are several tasks which you will be restricted from performing while on assignment. Although not considered all inclusive, these following tasks may NOT be performed at your work site: Do NOT attempt to pick-up or clean-up any broken BioHaz contaminated glass. Do NOT attempt to clean-up any hazardous chemical or BioHaz spill (even if you caused the spill); immediately notify your area supervisor. Do NOT attempt to fight any fire that might occur while at your work site, unless given specific authorization from Safety & Security during an unplanned fire event.

Corporate Job Bank – Workplace Safety Training Hazardous Materials Spill Prevention Hazardous Material Spill Reporting: • In the event of a chemical spill, immediately contact the supervisor in your assigned area; he/she will ensure proper documentation is completed to record the incident. • As previously mentioned, do NOT attempt to clean-up any hazardous chemical or BioHaz spills; personnel in your area have been trained to perform spill clean-up tasks; if necessary, contact Safety for further assistance. You can HELP through Spill Prevention: Housekeeping = Individual Responsibility a. All personnel should keep their work areas clean and uncluttered. b. Chemicals & hazardous materials must be properly stored and labeled. c. Work areas must be cleaned upon work completion and at the end of the shift/day. d. Any special cleaning problems or unsafe conditions should be reported to the supervisor.

Corporate Job Bank – Workplace Safety Training Compressed Gases OSHA & CAP regulations require compressed GAS cylinders be secured at all times: • Immediately report unsecured gas cylinders to your supervisor • Chains, sturdy straps or approved stands may be used to secure gas cylinders • Properly secured gas cylinders prevent serious accidents and injuries within the workplace • WARNING: If a large compressed gas cylinder is knocked over by accident & valve assembly is damaged, the container can quickly become an extremely dangerous projectile, capable of causing significant property and equipment damage, including the potential for severe injury or death!

  7. Corporate Job Bank – Workplace Safety Training Hazardous Communications or HAZ-COM “The Right To Know!” • The Hazard Communication Standard is a Federal Regulation. The regulations are found online (www.osha.gov): 29 CFR 1910.1200 • Basically, the regulations state that, as a contracted staff member, you have the “Right to Know” what hazards exist in the workplace. • This is accomplished several ways: • Material Safety Data Sheets (MSDS/SDS – Safety Data Sheets will replace MSDS effective 12/1/2015) for each chemical • Chemical Container Labeling • Warning Signs • Chemical Compatibility Procedures & Safety Training

Corporate Job Bank – Workplace Safety Training Hazardous Communications or HAZ-COM Identifying Hazards - Material Safety Data Sheets (MSDS) • Material Safety Data Sheets/Safety Data Sheets (MSDS/SDS) play a major role in Hazards Communication to the workforce • AnMSDS/SDS contains specific information and the hazardous ingredients • Required by law & Readily accessible to ALL workers • This document also gives information on proper handling & storage, disposal and harmful effects of the product (i.e. – Fire, Health, Reactivity, etc.) • OSHA Regulates chemical exposure limits (PEL, STEL, TWA) • First Aid if exposed & PPE requirements for handling • MUST be kept in department or work area where chemicals/reagents are being used!

Corporate Job Bank – Workplace Safety Training Hazardous Communications or HAZ-COM • Provides info & warnings to ALL employees & visitors • Helps emergency personnel determine best method for containing spills and/or fighting fires • Container Labels identify contents and show hazard(s) “NFPA” – Fire Diamond: • “National Fire Protective Association”– sets hazard ratings • Blue, Red, Yellow and White sections on a diamond • Hazards rated from 0 to 4 = (“None” up to “Extreme” dangers) BLUE = Health RED = Flammability YELLOW = Reactivity WHITE - Special information (such as do NOT use water, Bio-Haz radiation, etc.)

Corporate Job Bank – Workplace Safety Training Modes of Exposure to Chemical Hazards Inhalation • Breathing in contaminated air, fibers & chemical fumes/vapors Absorption • Absorbing what is spilled through the skin • Open cuts or breaks in your skin Ingestion • Though you’re unlikely to drink a hazardous substance, you could swallow something hazardous “if” it gets on your hands, then transferred by touch to food or beverage container Injection • Puncture wounds from objects that have been in contact with hazardous chemicals (i.e. – broken glass, various other sharps)

Corporate Job Bank – Workplace Safety Training FIRST AID for Chemical Exposure EYE WASH • Flush splashes to the nose, mouth or skin with water • Irrigate the eyes with clean water or a sterile irrigant for at least 15 minutes • Compliance with standard ANSI Z358.1-2004 • Wash hands frequently & always wash hands if they come in contact with chemicals! • For large area exposures, use emergency shower (a co-worker may require your assistance)! • For inhalation irritants (i.e. – fumes, vapors) remove employee to fresh air for at least 15 minutes and open doors/windows in room, if feasible. • Evacuate area if necessary! • Fill out proper report & follow-up with Occupational Health, if medical treatment is needed.

Corporate Job Bank – Workplace Safety Training • All contracted or temporary employees are responsible & accountable for their own personal safety. • Report workplace hazards to your immediate supervisor and/or manager and/or contact Corporate Job Bank with any safety concerns: • Morris Jessup, Vice President of Operations 480-966-0709 • Ensure prompt medical treatment for work related injuries, if necessary! All workplace injuries should be reported within 30 minutes. Corporate Job Bank contracts with Banner Occupational Health for evaluation and treatment of injuries sustained while on the job. If it is after hours, please leave a message at extension 105 with your name, details/time of injury, and your assigned employer. • Be aware of your surroundings. Act responsibly & safely at all times! • Be aware of safety procedures & policies that are relevant to your assigned work area (i.e. – use proper personal protective equipment, PPE, when applicable)
