Open Enrollment Basics Open Enrollment: November 1 – November 18, 2011 • It’s your once-a-year opportunity to enroll for new benefits or make changes to current benefits • Changes you make during Open Enrollment are effective January 1, 2012 through December 31, 2012 • No changes during 2012 unless you have a “qualifying event” IMPORTANT! • You must go online and enroll or waive coverage for the following in 2012: Vision, Health Care and/or Dependent Care FSA, Health Savings Account (for CDHP members only), and the new optional LTD buy-up plan. • Your Medical, Dental, and Legal plan elections will carry over to 2012.

What You Can Do During Open Enrollment • During Open Enrollment, you can: • Elect or waive health care coverage (medical/dental/vision) • Add or remove dependents • Elect to participate in a Flexible Spending Account (FSA) • Health Care – Standard or Limited Purpose* • Dependent Care • Elect to open a Health Savings Account (HSA)* • Apply for supplemental life insurance (subject to approval) • Purchase optional LTD buy-up plan • Purchase Hyatt Legal coverage *Only available if you enroll in the CDHP

What’s New for 2012 • Benefits eligibility modified to conform with Health Care Reform • You’re eligible if: • You’re a regular employee on the US payroll • You are scheduled to work 30 or more hours per week • If you’re eligible for coverage, you can also enroll your eligible dependents Eligible dependents include: • Legal spouse (unless he or she has access to an employer plan that costs less than $160 per month) • Children up to age 26 • Unmarried, biological children of any age who are disabled, as long as coverage began and the disability occurred before he or she turned age 26.

What’s New for 2012 • New medical plan provider — UnitedHealthcare • There are minimal changes to the plan designs • Prescription Drugs will still be with Medco • There are changes to: • The provider network — To find out if your current provider participates in the UHC network, go to www.myuhc.com/groups/tomkins and review providers in the Choice Plus network • ID cards — All employees and dependents enrolled in the plan will receive new ID cards by January 1st • Transition of care is available • For those in the middle of a course of treatment with a provider whodoesn’t participate in UHC’s network. • Form available on Powering Life website Why are we moving to UnitedHealthcare? UHC offers great provider discounts to save you and Tomkins money on medical services. UHC also has a broad national network of doctors and hospitals.

  9. What’s New for 2012 New benefit! — Basic Long-term Disability (LTD) Gates pays the full cost! If you’re benefits eligible, you’re automatically enrolled Replaces 50% of your base pre-disability earnings, up to $5,000 benefit per month Optional buy-up to 60% of your base pre-disability earnings, up to $6,000 per month Upon approval, benefit paid after you’re disabled 180 days For occupational or non-occupational injuries or illnesses Subject to pre-existing rules the first year of coverage Logon to the Powering Life website for more information LTD BUY-UP Enroll for buy-up LTD during Open Enrollment and EOI is not required!

Life and Accident Insurance Voluntary Life Insurance Apply on line for additional life insurance on yourself and/or your dependents You will be contacted by The Hartford in December to complete an Evidence of Insurability form The Hartford will be in contact with you about the status of your request, if more information is needed, and the final determination – this process may takes weeks/months AD&D coverage can be increased on line without proof of good health

Flexible Spend and Health Savings Accounts (FSA and HSA) • TaxSaver Administrator • Pre-tax Payroll Deductions to Reimburse Yourself with Tax-free $$ for Eligible Out of Pocket Expenses • Flexible Spending Accounts (FSA) • Contribute $260 to $5,000 annually • Health Care - Medical, Dental, Vision, Rx • Debit Card for On The Spot Payment • Limited Purpose Health Care Account – Dental and Vision Expenses for those electing HSA • Dependent Care – Child Care and Adult Care Expenses While You Work or Attend School • Health Savings Account (HSA) • Similar to health care FSA, but must be enrolled in the CDHP medical plan to participate • 2012 maximums = Individual - $3,100 Family - $6,250

Ready to Enroll? • Log on to the Powering Life website at www.smartchoicesgates.com to review your 2012 Open Enrollment Materials • Log on to Oracle Self Service from any computer with internet access at https://sshr.gates.com • User Name: gateshr • Password: accessonline • Follow the instructions in the Navigation Guide • Username is your employee # (if you are not a regular Oracle user) and can be found on your paystub • Password is year of birth and last four of SSN (YYYY####) (if it has been more than 90 days, since you last logged into Oracle)

Important Reminders • Open Enrollment ends on November 18, 2011 • Benefits are effective from January 1 thru through December 31, 2012 • You must logon and elect or waive Vision, FSAs, Health Savings Account, and Optional LTD buy-up plan. • If you have had a marital status change, be sure to update your Life and Accident insurance beneficiary designation (form online at the Powering Life website).
