  3. Workshop Goals • Familiarize applicants with the vision, mission, and strategies of Susan G. Komen for the Cure® • Inform potential applicants about funding opportunities • Help applicants develop a successful application • Encourage networking with other breast health professionals

  4. Vision • A World Without Breast Cancer • Promise • The Susan G. Komen for the Cure promise is to save lives and end breast cancer forever by empowering people, ensuring quality care for all and energizing science to find the cures.

  5. Central Texas Affiliate • One of more than 120 Komen affiliates worldwide • Started in 1999 with a close network of breast cancer • Activist • Service area of 6 counties and their cities • 75% of net funds raised by Affiliate stay in Central Texas

  6. Community Profile – Demographics and Breast Cancer Statistics • Breast cancer impact in our Affiliate area was measured by several key variables. In this analysis, we focused on mortality, incidence by stage of diagnosis, and screening as the most critical indicators of breast cancer impact. This evaluation provided the background and framework that indicated a priority need to focus support and outreach to three counties: Falls, McLennan, and Milam. After targeting these areas, expansion to all counties will occur. • Key factors for choosing Falls, McLennan, and Milam counties as target communities for the community profile project are as follows: • Ethnicity • Access to Care • Poverty/Education Levels • Under/uninsured

Community Profile – Health Systems Analysis • Komen Central Texas discovered through conducting these key constituent interviews that the biggest barriers discussed were: • Access to care • Lack of financial resources or insurance • Trust/fear • Education • Specific needs were identified: • More education/outreach to diverse communities (i.e., appropriate breast health educational materials, more speaking engagements, etc.). • Mobile Mammography availability to better serve the community. • Develop new relationships with community leaders and strengthen existing connections.

  8. Funding Priorities • Priority 1 • Increase the number of health services and providers available in the target counties by funding health system partnerships to increase access to services. Programs within this priority should include strong collaborative projects that include multiple partners to address the entire continuum of care.

Funding Priorities • Priority 2 • Partner with community-based outreach/health organizations, churches, and church leaders to effectively promote awareness of breast health, and further educate target populations about breast health services to include breaking down cultural, religious and language barriers for African American and Hispanic populations in Falls, McLennan, and Milam counties. Emphasis will be placed on programs that focus beyond awareness and incorporate education that is action based. Ex: A program that focuses on the individual making behavior changes or referrals to screening services

Eligibility • Non-profit organization • All past and current Komen-funded grants and awards are up to date and in compliance with Komen requirements • Project must be specific to breast health and/or breast cancer • Provide services in one or more of the following locations: • Bell • Bosque • Coryell • Falls • McLennan • Milam

Submission Process • Submit application “online” before 5 pm on December 17, 2012 • Required Information • Cover Page • Organization Summary and Project Abstract • Project Work plan • Program Description • Budget and Justification • Attachments • Promotional Items • Agreements with other facilities providing services/letters of collaboration ONLY if applicable • Copy of 501(c)(3) designation letter

Program Description • Brief explanation of project • Statement of need/problem to be addressed • Constituency • Goals and objectives (see example on next page) • Activities • Timetable for accomplishing goals • Collaboration • Evaluation plan • Organizational capacity • Sustainability • Plans to include recognition of the Central Texas Affiliate of Susan G. Komen for the Cure for providing funding for the program.

Project Description – Goals and Objectives • Objectives #1-3 (or more): Please Detail Specific Aims • Example for Objective #1: To provide breast health screening and education services to the uninsured African American women in Falls County to increase importance of breast health. • Anticipated Outcomes: at month 6, and at month 12 (be specific) • Example for month 12: 36 uninsured African American women will receive a Clinical Breast Exam (CBE) from a nurse practitioner along with a 15-minute breast health education presentation from the education director throughout the months of April 2013-March 2014 to help them make more informed decisions regarding their breast health. If there is a suspicious finding from the CBE, (or they are at the age that they should be getting a screening mammogram) we will work with x facility to setup that screening, as well as work with XY Organization to help with transportation reimbursement.

  17. Budget • Salaries and fringe benefits for staff working on the program. • Consultant fees • Meeting Costs • Supplies • Travel • Other direct program expenses • Will not accept indirect cost in the budget this year

Funds may not be used for the following purposes: • Medical or scientific research (costs associated with program evaluation are allowed) • Media projects (film, radio, web sites, public service announcements, etc.) • Scholarships or fellowships • Construction or renovation of facilities • Political campaigns or lobbying

  19. Supporting Documentation • Information regarding key personnel – For key personnel that are currently employed by the applicant, provide a resume or curriculum vitae. For new or vacant positions, provide job descriptions. 2 page limit • Proof of non-profit status – to document your federal tax-exempt status, attach/upload your determination letter from the Internal Revenue Service. Evidence of state or local exemption will not be accepted. Please do not attach/upload your Federal tax return. • Letters of support, collaboration, MOUS, etc.- demonstrating your partnerships • Evaluation forms, surverys, logic model, etc. - related to demonstrating the effectiveness of your program as defined in your work plan.

  20. Contracts • A grant contract will be the legal mechanism for funding. Please be advised that Grantee’s will agree to maintain the following insurance during the term of this Grant Contract:  • commercial general liability insurance with combined limits of not less than $1,000,000 per occurrence and $2,000,000 in the aggregate for bodily injury, including death, and property damage; and • excess/umbrella insurance, excess to the insurance set forth in (i) above, with a limit of not less than $5,000,000.  Grantee shall name Komen Affiliate as an additional insured under its commercial general liability insurance policy solely with respect to the Breast Cancer Project and any additional policies and riders entered into by Grantee in connection with the Breast Cancer Project.  • Upon execution of this Grant Contract, Grantee will provide a certificate of insurance evidencing the above coverage to Komen Affiliate. 

  21. Education Materials & Messages • Susan G. Komen for the Cure® is a source of information about breast cancer for people all over the world. To reduce confusion and reinforce learning, we require that grantees provide educational messages and materials that are consistent with those promoted by Komen for the Cure, including promoting the message of breast self-awareness and knowing your risks for breast cancer. Please visit the following website before completing your application and be sure that your organization can agree to promote these messages: http://ww5.komen.org/BreastCancer/BreastSelfAwareness.html. • Komen for the Cure grantees are eligible to receive preferred pricing for Komen educational materials.   22. Selection Criteria • Impact: Will the program have a substantial positive effect on breast cancer disparities and the priority area selected? How will your program change people’s lives? • Feasibility: How likely is it that the objectives and activities will be achieved within the scope of the funded program? • Capacity: Does the organization, Program Director and his/her team have the expertise to effectively implement all aspects of the program? Is the organization respected and valued by the target population? • Collaboration: Does this program enhance collaboration among organizations with similar or complementary goals? • Sustainability: Is the program likely to be sustained? Is the impact likely to be long-term?

Helpful Hints and Tips • Read the RFP carefully — follow ALL directions. • Be innovative, realistic, specific. • Write clearly, use active rather than passive voice. • Avoid jargon or acronyms. • Allow plenty of time. • Ask for What you need now • Do not always maintain the status quo • Consider how you will sustain this program year to year • Rejected Proposals
