  2. Objectives of Web Survey Tool • Data entry system for new nurse, preceptor, manager of preceptor, and site coordinator • Survey completion • progress management • Data compilation for • statistical analysis

  3. User Types • Study New Nurse • Study Preceptor • Nurse Manager • Control New Nurse • Control Preceptor (or Manager) • Site Coordinator

  4. Survey List Page • List of surveys will appear along with due date and expiration date (in ascending order) • Participants will be unable to open ‘Completed’ or ‘Expired’ surveys

  6. Survey Dates for Site Coordinators • Demographics and Outcomes Survey • Open date – start of Phase II • Due date – Initial Survey, at 12-Month Survey, and Final Survey • Expiration date – 3 months past due date

  7. Survey Dates for Study Participants • Open date – date when survey is made available to user • Due date – date when survey is due for completion • Expiration date – grace period for survey completion; survey is no longer available after this date

  9. Survey Access • Each user has individual password-protected access • No user can view previously submitted surveys • Unable to view individual-level data for other participants • Study preceptors, site/state coordinators can view progress of survey completion for each user assigned to them
