MNM Fatal 2007-09

This incident report details a fatal fall of a 52-year-old laborer at an alumina operation in Arkansas. The victim fell approximately 57 feet from a scaffold while passing materials to a coworker. The root causes were inadequate management policies and procedures for scaffold erection and failure to ensure proper use of fall protection. Best practices include discussing work procedures, identifying hazards, ensuring floor openings are covered, securing safety lanyards, and providing proper training for restraint devices.
