  11. 2014-2015 Final Reporting • Required reporting: • Post SWI • Final Use and Impact Report (aka, the Final Report) • ADP and Dairy Sales (milk and yogurt) • Timing • Email to 2014-15 Funds Schools week of May 4 • Online report available Wednesday, May 6 • Due date: Tuesday, June 2 • District applications • Streamlined format – one report covers all schools • Individual ADP and Dairy Sales required from each school • Email to districts week of May 11 • Online report available Monday, May 18 • Appreciate S/R reminders to all funded schools

  12. 2015-2016 Funds Program What’s the same? • 2 Deadlines: • Tuesday, June 16 • Wednesday, Nov 4 • Fast-tracking and district applications available • Up to $4,000/year • Schools can apply for funding in multiple years • Amount requested for PA may not exceed amount requested for HE • Both HE and PA required for applications • Choose one of each Play from the 2015-16 Playbook • Online application format same as last year; schools first select Plays to generate application • MMS will continue to send notifications to schools

  13. 2015-2016 Funds Program What’s Changed? • No SWI required for applications; now optional • Scaled back pre-screening of applications; quality control review • S/R review remains the same • Edits to application • Removed question: • Which years has your school been enrolled in FUTP 60? • Revised question: • How you will track the impact of the Healthy Eating and Physical Activity Plays you outlined above? 

  14. Application Launch • Online application launches: Tuesday, April 21 • Application pdf on Team Dairy • November application will launch in mid-August • Application hiatus June to mid-August

  15. 2015-2016 Funds Program Consideration for November Round: Reserve funding for grants in Hispanic Schools to coincide with Fall launch of Fuel Up to Play 60 enEspañol.

  16. Collaborators for June Round • No national collaborators for June Round • Regional funding available in some locations • Funding awarded for all approved applications; no equipment offers on national basis

District/Fast Track Applications • Launching in mid May • Streamlined process: • Participating district schools apply for same Plays • District completes one application with district-level questions and school-level questions • Submission of applications done all at once by the district • Review process streamlined • To start a District or Fast Tracked Application, fill out the District Starter Form and send to Jessica Harms • Accounting disperses check approximately 10-15 business days after Application is approved • District notification letter does not appear on Dashboard, must be sent directly to school via approval letter template
