  3. Civil Rights: Staff Training When should new staff be trained on Civil Rights? • When they’re being interviewed • Upon employment • After employment - when all the staff receive their annual training

  6. Civil Rights: Data Collection Form • Civil Rights Data Collection to be completed with November enrollment/attendance • Institutions are required to obtain the race and ethnicity of program participants upon enrollment and annually thereafter

Civil Rights: Data Collection Form: Part 1 Completing the racial and ethnic data for all enrolled participants at your facility: *Total enrollment or attendance MUST equal total number of ethnicity information; however, racial information may be equal or higher than total enrollment or attendance.

Civil Rights: Data Collection Form: Part 1 Racial Categories American Indian or Alaskan Native:A person having origins in any of the original peoples of North and South American (including Central America), and who maintains tribal affiliation or community attachment. Asian:A person having origins in any of the original peoples of the Far East, Southeast Asia, or the Indian subcontinent, including, for example: Cambodia, China, India, Japan, Korea, Malaysia, Pakistan, the Philippine Islands, Thailand, and Vietnam. Black or African American:A person having origins in any of the black racial groups of Africa. Native Hawaiian or Other Pacific Islander:A person having origins in any of the original peoples of Hawaii, Guam, Samoa, or other Pacific Islands. White:A person having origins in any of the original peoples of Europe, the Middle East, or North Africa.

Civil Rights: Data Collection Form: Part 2 Where to obtain census data for completing Part 2: • If your institution serves participants throughout the District, check “District.” You will need to gather data on the entire District since this is your population of potentially eligible participants. • For District data, go to: http://quickfacts.census.gov/qfd/states/11000.html • If your institution primarily serves participants from the ward in which the institution is located, check “Ward” and specify the ward. You will need to gather data on that ward since this is your population of potentially eligible participants. • For data by ward, go to: http://planning.dc.gov/DC/Planning/DC+Data+and+Maps/DC+Data/Census+2010+Data/Ward+Data+by+Race+and+Ethnicity+2000+&+2010
