Tofacitinibusesanditssideeffects • Tofacitinibisusedtotreatrheumatoidarthritis(RA)whenothertreatmentsdonotwork. • Itisalsousedtotreatactivepsoriaticarthritis(PsA)whenothermedicinesdonotwork. • Thiscanbealsorecommendedtousedinthetreatmentofmoderatelytoseverelyactiveulcerativecolitis(UC)whenothermedicinesdonotwork • WhatTofacitinibdoes? • ThedrugisbelievedinordertointerferewiththeactivityofanenzymeknownasJanuskinase(JAK),whichmainlyactivatesothercellularcomponentsthatnormallyinitiatetheimmuneresponseinthebody.Byreducingtheimmuneresponsexeljanz5mgreducesthesignsandsymptomsofrheumatoidarthritisandpsoriaticarthritis.Italsoreducesthesignandsymptomsofulcerativecolitis. • SeriousWarningandPrecautions:XeljanzisbasicallyakindofdrugwhichisresponsibleinordertoaffecttheimmunesystemandcandecreasetheabilityofthebodytofightoffinfectionssuchasTB(tuberculosis),andotherinfectionsthatcausedbyfungi,bacteria,orvirusesthatcanspreadthroughoutthebody.Theseinfectionscanberesponsiblefordeathorhospitalization.Mostpatientswhodevelopedtheseinfectionsweretakingothermedicinesthatmakeithardertofightinfectionsatthesametimesuchasmethotrexateorcorticosteroids.Youshouldnotbeusingitifyouhaveanysortofinfectionrelateddisease.  doctor.  Ifaseriousinfectiondevelops,donottakeit,andimmediatelydiscusswithyour Yourdoctorwillcloselymonitoryouforthesignsandsymptomsofinfectionduring andafterthetreatmentwiththismedicine.  Lymphoma,someothercancersandalsootherseriouseventshavebeenobserved • inpatientswhoweretreatedwiththeXeljanz • Bloodclotsintheveinsofyourlegsorarms(deepveinthrombosis,DVT),arteries(arterialthrombosis)orlungs(pulmonaryembolism,PE)canhappeninsomepeoplewiththisdrug.Thismaybelife-threatening. • Ifyoudevelopanysignsorsymptomsofabloodclotinyourleg(suchasswelling,painortendernessintheleg)orinyourlung(suchassuddenunexplainedchestpainorshortnessofbreath)stopTofacitinibandseekimmediatemedicalhelp. • Interactionswiththismedication:ItiscrucialthatyourhealthcareprofessionalbeawareaboutallthedrugsormedicationsyouareconsumingpriortoinitiatingthexeljanzincludingbiologicssuchasCimziaTM,Enbrel,Cosentyx,Humira,Kineret,Remicade,Orencia,Rituxan,Entyvio,SimponiTM,andStelara. • Tellyourhealthcareteamifyouareconsumingimmunosuppressants(e.g.azathioprine,6-mercaptopurine,tacrolimus,sirolimus,cyclosporine),antiarrhythmics,beta-blockers,calciumchannelblockers,cholinesteraseinhibitors,HIVproteaseinhibitors,rifampin,ketoconazole,fluconazole.  Tellyourpharmacistordoctorifyouhavetakenanyvaccines(shots)within1month • priortoinitiatingthismedicine. • Avoidgrapefruitjuice.

  2. St.John’sWort(anherbaldrugalsoknownasthehypericumperforatum)could reducetheresponsetoXeljanz. Whenitshouldnotberecommended:Ifyouareallergictoitoranyothernon-medicinalingredientsinxeljanz(Tofacitinib),youshouldnottakethisdrug.Youshouldavoidthismedicationifyouarepregnantorareplanningtobecomepregnant.Donotconsumethismedicinewhilebreast-feedingorintendtobreast-feed.Talktoyourdoctorabouthowtofeedyourchildwhile takingTofacitinib5mg.Patientswithsevereliverinsufficiencystrictlyavoidtheconsumptionofthismedication. Contraindications:Tofacitinibiscontraindicated:    Inthosepatientswhoarewithknownhypersensitivitytothismedication.Inpatientswithseverehepaticimpairment. Duringpregnancyandbreastfeeding. SideEffectsofTofacitinib: Thesearenotallthepossiblesideeffectsyoumayfeelwhentakingthismedication.Ifyouobserveanyadverseeffects,whicharenotlistedhere,talktoyourhealthcareprofessional.ThesideeffectsofXeljanzinclude:                  RashCoughDizzinessVomitingBackpainHeadacheDiarrheaJointpain MuscleweaknessPain UpperrespiratorytractinfectionsNausea IndigestionUpsetstomachNasopharyngitis NoseorthroatinfectionRunnyorstuffynose Overdose:Bymistake,ifsomeoneconsumedanoverdose,immediatelydiscusswiththeirhealthcareprofessional,regionalPoisonControlCentre,ornearesthospitalemergencydepartment,eveniftherearenosignsandsymptoms. MissedDose:Ifyouhavemissedyourdose,takethenextdoseasplannedatthenextscheduledtime.Neverconsumeacoupleofdosesinordertomakeupfortheforgottendose. TofacitinibCost:Thecostoftofacitinibisquiteaffordable.Arespectivepatientcanpurchaseitthroughanylegitimatepharmaceuticalswholesalers.Itshouldbeconsumedwithorwithoutmeals.
