Health Educator/Project Recruiter • Menominee Indian Tribe of Wisconsin • HealthEducator/Project Recruiter • TribalClinic • ClassDescription#12912Grade9Non-exempt Revised11/23/05 • GENERALDESCRIPTION OFDUTIES • UnderdirectsupervisionoftheDiabetesPrimaryPrevention,Nutrition/WIC,Director,thepurposeofthepositionis aseducatorfortheMenomineeTribalClinic’sSDPICompetitiveGrantsProgramDiabetesPreventionDemonstrationProject,includingdeveloping,implementing, andevaluatingadiabetespreventionlifestylecurriculum,participatinginthedevelopment andimplementation of relatedclasses,demonstrations, andcommunityandfamily-basedactivities, andensuring thatall programmaticactivitiesarecarriedoutinaccordancewithgrantguidelines,project protocols, standardsof care,andpoliciesandproceduresoftheMenomineeTribalClinic. • EXAMPLESOF ESSENTIALFUNCTIONS • IncollaborationwiththeProject Coordinator andthelocaldiabetesproject team, aswellaswiththeCGP CoordinatingCenter, IHS,othergrantees,otherhealthprofessionals, andcommunity partner,theHealthEducator/Project Recruiter: • Participateinthemodificationanddevelopment ofthelifestylecurriculumand educationalmaterialswithothergrantees, localproject team members, andappropriatelocal communitymemberstoincorporatecollaborativedecisionsoncurriculum, eligibility criteria,evaluationmeasures, andlocalculturalvalues. • Implement thecurriculum, conductingsessionsor coordinatingandmonitoringother staff inconductingsessionsaccordingtotheagreeduponcurriculum, project protocols, and local culturalvalues. • Providediabetespreventiontrainingandeducationtoindividualparticipants, family members, communitygroups, andotherproject staffonallaspectsofdiabetes care as neededorrequiredby theproject protocols. • Administer class evaluations,monitor dataforareasneedingimprovement, and implement performanceimprovement activities. • Procure, organize, evaluate, update,anddeveloppatient educationmaterialsforuseby healthcareprofessionals, patientsandfamilymembers, andcommunity-basedgroups. • Assist theProject Coordinatorinthedevelopment ofarealistic, measurableworkplanof interventionprojectactivities. • Assist inpreparingprogressanddatareports; assistinensuringtheiraccuracyandtimeliness insubmissiontotribalreview, IHS,andtheCoordinatingCenter. • Support andparticipateintheconceptualization,design, anddevelopment ofall project protocols,procedures, educationalanddatacollectionactivities, andinfrastructure systemsfor successful implementationandevaluationoftheproject accordingtogrant guidelines, tribalrequirements, andlocalculturalvalues. • Ensure thatallprogramactivitiesareperformedwiththeutmost attentiontopatient confidentialityandHIPAAandIRB requirementsforhealthcare researchand programming. • Participateactivelyinallnational,regional,andlocalGranteeproject development and trainingmeetings. • Maintaincollaborative relationshipswithgranteeteammembers, CoordinatingCenter DataCore staff, TechnicalAssistanceExpert Consultants, tribalandcommunitypartners, • 1

HealthEducator /ProjectRecruiter • andotherproject stakeholderstofoster successfulproject development andimplementation. • Assess communityhealthproblems, planappropriate,creative, dynamichealtheducationinterventions, successfullyimplement theprograms, andevaluatetheprocessandoutcomesoftheeducationalprogram. • Establishand maintaincooperative, supportiverelationshipswithproject participants, as wellasfundingagencies, clinic staff, tribalprograms, andlocal,state,andnational organizations. • Developfullunderstandingandskillinuseofallprotocols, procedures, eligibilitycriteria, andstudymeasuresrelatedtothecurriculumofboththeintensiveandlessintensiveinterventions. • Useavarietyofteachingstrategies, approaches, and methodsthat incorporatetheories andconceptsofadultlearning, readinessfor change, empowerment andmotivational interviewing,andlocalculturalvalues. • Performrelateddutiesasdirected. • MINIMUM TRAININGANDEXPERIENCE • MinimumBachelorsdegreeinWellness,orotherrelevanthealthorcommunityhealtheducationfield;CertifiedDiabetesEducator,orCertifiedHealthEducationSpecialistpreferred.Experiencewithdirectpatientcare(throughcareandeducationforpatientswithdiabetes)preferred. • SPECIALREQUIREMENTS • Ability toapplyspecialized knowledgerelatedtothecare ofpatientswithpre-diabetes indiverse communities. • Knowledgeoftheprinciples, practices, andstandardsofcare forpre-diabetes/metabolic syndromediagnosesrecommendations, diabetes healthpromotion,diseaseprevention, and treatment. • Knowledgeofeducationalprinciplesofadult learningtheory,groupdynamics, behavior changeconcepts, empowermentmodels, motivationalinterviewingtechniques, learningstylesandteachingstrategies.Ability tocommunicateeffectively withavarietyofaudiencesusingavariety of mediums. • Groupfacilitationskillswhichkeepgroupsontasksandsensitivetoprocessandissues. Understandthesocialandculture context ofAmericanIndian/AlaskaNativecommunities servedbytheMenomineeTribalClinic.Abilitytointegratethisknowledgeofthecommunities intoproject activities. • Excellentoralandwrittencommunicationskills; highlyorganized. • Excellent computer skillsincludingword processing,email, internet, spreadsheet,anddatabaseusage. • Willingnessandabilitytotraveltocollaborativesitesandnationalproject meetings. • Maintainprofessionalcertificationand/orlicensurefromanationallyrecognizedcredentialing body.

HealthEducator /ProjectRecruiter Maintainaminimumof6professionalcontinuingeducationcreditsannuallyontopicsrelatedtodiabetesmanagement, CVD riskreduction,behavioralinterventions, teachingandlearningor counselingskillsworkshops, andin-serviceprogramsrelevanttodiabetesindiversecommunities. PERFORMANCE APTITUDES DataUtilization:Requirestheabilitytoperformmid-leveldataanalysisincludingtheabilitytoaudit,deduce,assess,concludeandappraise.Requiresdiscretionindeterminingandreferencingsuchtoestablishedcriteriatodefineconsequencesanddevelopalternatives. HumanInteraction:Requirestheabilitytopersuade,convince,influence,train,andmonitor,infavorofadesiredoutcome.Requiresabilitytoactasaleadperson. Equipment,Machinery, Tools, and Materials Utilization:Requirestheabilitytooperate, maneuverand/orprovidesimplebutcontinuousadjustmentonequipment,machinery,tools, and/ormaterialsusedinperformingessentialfunctions. Verbal Aptitude:Requirestheabilitytoutilizeawidevarietyofreference,descriptive,and/or advisorydataandinformation. Mathematical Aptitude:Requirestheabilitytoperformaddition,subtraction,multiplicationanddivision;abilitytocalculatedecimals andpercentages;mayincludeabilitytoperformmathematicaloperationswithfractions;mayincludeabilitytocomputediscount,interest,profit andloss,ratioandproportion;mayincludeabilitytocalculatesurfaceareas,volumes,weights, andmeasures.
