Title of the Project Last name, First Initial1, Last name, First Initial2* (corresponding author is marked with an asterisk) 1 Address for the first author, including the institution ; 2 Address for the second author, including the institution * Email address of the corresponding author (preferably the VEP) BACKGROUND Justification Include here a brief overview of the justification for your project and any relevant information related to the disease, distribution, population of interest, etc. Use formatting to help set the sections apart For example, begin each section with a title which is bolded and a different color than the text OBJECTIVES Specifically and concisely state the objectives of your project MATERIALS & METHODS Study Population Include a description of your target population and study population Sampling How were units sampled (criteria for inclusion or exclusion)? What was your list frame? How many units were sampled in all? Questionnaire design How did you design your questionnaire? What were the sections included? What were the specific items that the questionnaire was designed to measure? How was the questionnaire piloted or assessed prior to use? Questionnaire administration How was the questionnaire administered? How was the data collected and entered into the computer? Laboratory analysis Include a description of the laboratory analysis used. Statistical analysis How was the data analyzed? RESULTS Below are some optional categories you may want to consider in the response section. Survey response What was the response rate for your survey? Did anyone decline to participate? If so, why? Respondent demographics What are the characteristics of the people who responded to your survey? You may want to include a map showing the distribution of respondents by region or town. Results from your survey sections Present the results from your project. You may want to use graphs or pie charts to help display more complex information. I would recommend creating the charts or graphs in excel and them cutting and pasting them into powerpoint when you have them the way that you want them. Each chart or graph needs a title that clearly explains what is contained in the chart or graph. Be consistent in where you place your titles – either above or below the graphic. Chart 1. CONCLUSION & DISCUSSION Discussion Include some discussion about the relevance of your work, additional work that needs to be done or is on-going, how this project´s results will impact things in your country either in communication or policy, etc. References You may include a brief reference section (no more than 3 as a general rule of thumb) Acknowledgements You may also want to include some brief acknowledgements
