  1. Procedure for Fitness-for-Duty Requests Department Supervisor or Director has consulted DPM or NCM and verified that FFD is indicated • Director or designee must send memo specifying: • Reason for request • Support for the request with examples of behavior or performance issues • Copy of the essential job functions • Name/# of contact person • Must ask EME to determine if long or short term issue and what, if any, limitations • Must be copied to the employee Director or designee sends FFD request to EME to assure compliance with regulations. May come addressed to NCM or DPM. Is request in order? NCM or DPM contacts sender with recommendations NO YES EME discusses with NCM and/or DPM. NCM reviews record, obtains other medical information as necessary and prepares a summary NCM discusses with EME & assures the FFD is scheduled & notifies Supervisor of time. Supervisor notifies employee. Possible determinations: • Fit for full duty • Short term impairment [ Light Duty] with specified limitations • Significant impairment – no duty • Long term condition with limitations – may need accommodation EME evaluates employee and makes determination
