  2. What is our goal? Our goal Why we are having this symposium Past successes –North Dakota and Nebraska Ultimately we want to inform and educate The backdrop? Implementing an electronic health record is the single most complex and resource intensive business process any health care provider can undertake!

  3. Success stories-notes from the field Success stories There isn’t a cookie cutter approach! Since 2005 (our first EHR presentation at CRHC) we have advocated for the “triple play” Needs Assessment, Readiness Assessment and Workflow analysis You can use this approach even if you already have an EHR!

  4. The needs assessment Needs assessment A great way to involve all stakeholders Early identification of champions Often “needs” are business and clinical process changes rather than software needs A way to validate stakeholders and build enthusiasm A way to identify if more than one system or solution is needed (hospitals-ED or clinics a disease registry)

The readiness assessment Readiness assessment Determine the cultural readiness Are there competing corporate priorities Financial review Key system review-finance and billing, laboratory and pharmacy Computer literacy Chart and data conversion IT infrastructure Corporate support Can you afford an EHR?

Workflow Analysis and redesign Workflow analysis Look at the typical patient encounter and care processes Each has a data input and data output component Each has a human intervention component How will these change with an EHR? Identify templates early in the process Or where an interface is necessary (lab or diagnostic equipment) Thursday’s Workflow presentation

How these steps correlate with the REC Rec correlation Readiness assessments are a key initial step Vendor selection and contracting Vendor implementation management Workflow analysis and redesign to achieve meaningful use-in some cases our scope will “creep” to incorporate key processes that support a successful EHR Facilitating interfaces and interoperability

REC correlation-Cost analysis Cost analysis We will help develop a multi-year cost analysis Our focus is on direct costs AND indirect costs Key focus areas include data conversion and any software customization needed For example productivity reduction, infrastructure enhancements, insurance and so forth We will also work with you to determine incentive funding

  9. Vendor evaluations scenarios The power of a scenario driven approach Please refer to the ambulatory and hospital scenarios These correlate to meaningful use and also to basic CCHIT certification however they primarily focus on routine functionality

  10. Vendor Evaluations evaluation The value of a needs assessment and workflow analysis is to identify key scenarios for your organization!

Sample evaluation program for internal use evaluation Use a simple scale like: A=Excellent B=Good C=Passable D=Insufficient/Not Good Evaluation Criteria Presenter was prepared and presented the implementation plan based on our unique environment Presentation was coherent and easy to understand Presentation demonstrated an understanding of the details and scope of the practice management conversion

Evaluation evaluation Presentation demonstrated an understanding of the details and scope of the existing data conversions (charts, registration data, finance data, scheduling information and so forth) Presentation demonstrated an understanding of the details and scope of the existing transcription conversion Presentation demonstrated an understanding of our physicians and providers preferences for entering medical records (dictation, point and click etc.) Presentation demonstrated a training program that we believe will work Presentation acknowledges sufficient time for template design and system adoption

  13. Evaluation evaluation Presentation acknowledges any additional hardware resources we may need and when they should be installed After the presentation I could understand the various stages or phases of implementation
