  20. CRITICAL ILLNESS RIDER Min. Age at entry: 18 years Max. Age at entry: 45 years Max. cover age: 55 years Duration: 5 years or 10 years (should not exceed the term of basic cover) Sum Assured: 18 to 35 yrs: Rs. 3 lac or Rs. 5 lac (should not exceed the basic cover) 36 to 45 yrs: Rs. 3 lac Premium: Gender, Age, Duration, amount of cover. Premium rate: • valid only upto 31st March 2008 • SBI Lifereserves the right to revise the premium thereafter. If Critical Illness cannot be considered without medical: The policy will be issued without C.I. cover – Premium paid towards CI cover will be refunded

CRITICAL ILLNESS benefits Exclusion period 6 months. • When a critical illness is diagnosed: • Policyholder informs SBI Life within 30 days • S.A. under this rider is payable if the policyholder survives for more than 30 days. • No more benefits will be payable from this rider. • Policy still continues for the basic cover. • Survival benefit: • No survival benefit No bills to submit: On diagnosis, S.A. for C.I. will be paid

  30. CANCER • A malignant tumour characterized by the uncontrolled growth and spread of malignant cells and invasion and destruction of normal tissue. The diagnosis must be histologically confirmed. The term cancer includes leukaemia but the following cancers are excluded: • All tumours which are histologically described as pre-malignant, non-invasive or carcinoma in situ; • All forma of lymphoma in the presence of any Human Immuno deficiency Virus; • Kaposi’s Sarcoma in the presence of any Human Immunodeficiency Virus; • Any skin Cancer other than invasive malignant melanoma • Non life threatening Cancers, such as Prostatic Cancers which are histologically described as TNM classification T1 or are of another equivalent or lesser classification • (back)

  31. Coronary Artery Bypass Surgery The actual undergoing of open chest surgery for the correction of two or more coronary arteries, which are narrowed or blocked, by coronary artery bypass graft. Angiographic evidence to support the necessity of the surgery will be required. Angioplasty and/or any other intra-arterial procedures are excluded from the cover . (back)

  32. Heart Attack • Means the death of a portion of the heart muscle arising from inadequate blood supply to the relevant area. The diagnosis shall be supported death of a portion of the heart muscle arising from inadequate blood supply to the relevant area. The diagnosis for this will be evidenced by all of the following criteria:shall be supported if all three of the following five criteria are present: •   History of typical chest pain • Confirmatory new electrocardiogram (ECG) changes, • Elevation of infarction specific enzymes. • Non –ST segment elevation myocardian infarction (INSTEMI) with elevation of Troponin I or T is excluded.(back)

Kidney Failure End stage renal failure presenting as chronic irreversible failure of both kidneys to function, as a result of which either regular renal dialysis or renal transplant is undertaken. Evidence of end stage kidney disease must be provided and the requirement for dialysis or transplantation must be confirmed by a consultant physician. (back)

  34. Stroke Any cerebrovascular incident producing neurological sequelae lasting more than 24 hours and including infarction of brain tissue, haemorrage and embolisation from an extracranial source. Evidence of permanent neurological damage must be confirmed by a neurologist at the earliest 6 weeks after the event and no claims can be admitted earlier.Cerebral symptoms due to migrain, cerebral injury resulting from trauma or hypoxia and vascular disease affecting the eye or optic nerve as well as ischaemic disorders of the vestibular system are excluded.   35. Major Organ Transplant The actual undergoing as a recipient of a transplant of a heart, liver, lung, pancreas or bone marrow as a result of chronic irreversible failure. Evidence of end stage disease must be provided and the requirement for transplantation must be confirmed by a consultant physician. (Back)
