Background • Condition Management Practitioners for the Pathways to Work Project since 2004 • Previously worked in mental health services collectively for a period of 34 years • We have extensive experience of working in a variety of hospital and community settings, promoting the rehabilitation and recovery of clients with mental health problems

Condition Management – “A Unique Experience” Why unique? … practitioners found working in CMP much more rewarding than any past experiences What makes it unique? … practitioners are empowered to be innovative and creative in their practice, well supported by management team How do we find this out? … questionnaires were issued to colleagues looking at factors cited as being important to them

  4. We hope that these findings provide a flavour of the factors we consider important in providing an effective condition management programme

  6. Q. What influenced practitioners to join the team? • Non medical model • Shared vision and philosophy • Emphasis on improving customer lifestyle • Opportunity to use self help techniques • Freedom to use skills and knowledge unused in previous positions

  7. Autonomy and Flexibility • Therapeutic use of self • Voluntary for customer • Partnership working – with Non NHS partners

What was considered to be different about working as part of the Condition Management Team? • Without the traditional hierarchy system that exists in traditional NHS setting • Truly a multi-disciplinary team who freely share skills and knowledge • Future orientated – more emphasis on solutions

  9. Forward thinking – the opportunity to try new ideas and to develop new skills through training • Short term intervention • Degree of autonomy, freedom to practice and use own clinical judgement

  10. Q. Do you consider that the relationship with your customers is different? If so, why? • Based on wellness • Evidence based, individually tailored to customers needs • Supported Self Help v treatment • More emphasis on education • Short term intervention

  11. Looking at customers whole life – not just work • Flexibility to use a range of methods and approaches incl. complimentary therapies • Normalisation – use of existing community resources

  12. Q. Have CMP’s reported higher levels of job satisfaction since joining team? 100% YES

  13. Q. Why do you think this is the case? “Able to use and develop skills, learn new skills, feel more motivated” “Interventions seem to be more successful, also short term input allows for a fairly rapid turnover of customers” “The autonomy to practice in the best way to suit individual needs”
