  4. First Report Due – 2012 FACT-GP Assessment & HH Functional Questionnaire Data

  5. 2012 Submission Data Requirement What to Submit • FACT-GP Assessment / HH Functional Questionnaire data from calendar year 2012 is due Monday, May 13, 2013 • This first report will require only 2012 data collected for the FACT-GP Assessment / Health Home Functional Questionnaire for current members active in 2012 or for those that are now ‘case closed’ but were active in 2012. • After the member completes the FACT-GP Assessment /HH Functional Questionnaire, the answers are scored using the Scoring Guidelines for the FACT- GP /Health Home Assessment and FACT - GP and Health Home Questionnaire Scoring Sheet.

  6. 2012 Submission Data Requirement • The scores are entered into the 2012 Excel Spreadsheet template.( See next slide) • If a FACT-GP Assessment /HH Functional Questionnaire has not been completed for the 2012 reporting period for a member, then no 2012 data submission is required. • All subsequent HH CMART reports include ALL HH CMART data elements, and are due no later than the first Monday of the second month following the end of the reporting period.   7. 2012 Data-Excel Spreadsheet Template • 2012 data submission includes only elements # 2, 4, 7 and 35-49 of the Technical Specification’s document. • Below is an example of the 2012 partial Excel spreadsheet template showing the required elements for the 2012 FACT-GP Assessment / HH Functional Questionnaire data file due May 13, 2013. How to submit • 2012 data will NOT be entered into or require submission of the HH-CMART database tool. 2012 assessment data & scores will be entered into the above Excel spreadsheet template and submitted by the lead Health Home via the Health Commerce System (HCS) • Health Homes will send file using the HCS Secure File Transfer application to ‘Laura Morris’

  8. 2012 FACT-GP Assessment / HH Functional Q. on CM TCM Legacy Converting Clients • If the Care Management Program has converted to Health Home rate codes, the FACT- GP Assessment and HH Functional Questionnaire were required to be completed on all active and previously active clients. If you have not submitted information to the Health Home you must do so immediately.* • If you completed both assessments in 2012, then those scores should be put on the Excel spreadsheet template and submitted to the lead Health Home to which the clients have been assigned. • If you did not complete them in 2012, you cannot submit the data to the Health Home. Starting in 2013, collect complete HH CMART data on all active clients (Legacy or Health Home) from here on out . • Then, report all the HH-CMART data to the lead Health Homes for 2013 using the method/format agreed upon with the Health Home.* • *Note: Sharing of data is permitted after an agreement is in place

  9. 2012 Required data, Element Definitions • 2 – HHID: This is the MMIS number and will be the same for all members in the Health Home’s file. • More than one FACT-GP + HH Questionnaire in 2012 – if a member had more than one assessment in 2012, use the results from the earliest date the assessment was completed as we are looking for the initial or baseline assessment. • More than one Care Management Provider - If a member changes care management providers during the reporting period, the Health Home should include the results from the earliest assessment in 2012. For example, if the member did an assessment with CM Agency A in May 2012 and another with CM Agency B for the August 2012, the results from the May assessment should be included in the 2012 file for the Health Home. • More than one Health Home - If the member changes Health Homes, the CM provider should include the FACT-GP +HHQ results for the earliest assessment in the file for the Health Home the member was engaged with at the time (or closest to the time)of the assessment. For example, a FACT-GP +HHQ is completed in June 2012. The member was engaged in Health Home One from April to August 2012. The member is engaged in Health Home Two from September to December 2012. The June 2012 assessment results should be included in the file for Health Home One.If another FACT-GP+HHQ is completed between September and December, this result should be included in the file for Health Home Two. If only one FACT-GP +HHQ is completed in 2012, it should be included in both Health Home files.

  10. 2012 Required data, Element Definitions • 4 – CIN: Medicaid Client ID Number-A valid CIN must be provided for every record and should be the CIN from the reporting period. • 7 – Date of Birth:of member, format is mm/dd/yyyy • 35 –DateFactHH:Date that the FACT-GP Assessment +HH Functional Questionnaire were completed • 36 –ReasonFACTHH: Indicates the reason the FACT-GP Assessment +HH Functional Questionnaire was completed • Response Value of either Initial, Annual, or Discharge

  11. 2012 Required data, Element Definitions – FACT- GP Assessment & HH Functional Questionnaire • Each section of FACT- GP questions result in a subscore • Subscores are put on the Excel Spreadsheet template for each appropriate element • 37 – PWB: Physical Well Being subscore • 38 –SWB : Social Well Being subscore • 39 – EWB: Emotional Well Being subscore • 40 – FWB : Functional Well Being subscore • 41 – FACTGP : FACT- GP Total Score –All Subscores are added to get the total FACT- GP score

  12. 2012 Required data, Element Definitions – FACT- GP Assessment & HH Functional Questionnaire • HH Functional Assessment Scoring- each answer is scored, and the score is recorded for each correlating element on the Excel spreadsheet template. • 42 – HH1: Health Home question # 1 Score • 43 – HH2: Health Home question # 2 Score • 44 –HH3: Health Home question # 3 Score • 45 – HH4: Health Home question # 4 Score • 46 – HH5: Health Home question # 5 Score • 47 – HH6: Health Home question # 6 Score

  13. 2012 Required data, Element Definitions – FACT- GP Assessment & HH Functional Questionnaire • 48 – HHSubscale: Scoring sheet adds the HH Functional Assessment scores together to come up with the HHSubscaleScore • 49 – HHFACT GP: Scoring sheet adds the Health Home FACT-GP Total Score + HH Subscale Score to come up with the Total FACT- GP / HH Functional Assessment Score

Question and Answer Themes • Distribution of the Excel template for 2012 data submission – The excel template was emailed to those lead Health Homes on the Departments HH CMART Master distribution list. • If you have not received the template please contact the lead Health Home to arrange for submission of the 2012 data . • If needed, email the Department’s Health Home program at hh2011@health.state.ny.us

Can the FACT-GP/Health Home Functional scoring be tabulated straight forward by adding the scores? • No, the FACT-GP is a research validated tool developed by FACIT for assessing quality of life • We are not allowed to make changes to the questions or the scoring due to its prior validation. • FACT- GP Assessment / HH Functional Questionnaire on clients that are ‘case closed’ – If a case has been closed and you have the data collected, then submit the assessment data on all former and currently active Health Home members (i.e. case re-opened) to the assigned Lead Health Home • Examples: • If a FACT- GP Assessment and HH Functional Questionnaire are completed by the member, and later in the quarter the case is closed, report the scores to the lead Health Home. Submit also if case had been closed and is now re-opened. • If the case is closed before the member completes the FACT-GP Assessment and HH Functional Questionnaire, then the scores cannot be reported. Question and Answer Themes • FACT-GP & HH Functional scoring influence on acuity score – The results of the FACT-GP Assessment and HH Functional Questionnaire will not directly impact the acuity score of the member. We are collecting this information and will be using it to get a more global sense of the population, including understanding the overall health of the population. We will be evaluating the data to determine its future utility in evaluating our members. • Homelessness scoring on the FACT-GP/HH Functional – Homelessness is scored on a Yes and No basis. A higher score represents better health, a score of “0” indicates that the member was homeless in the last 7 days; if they are housed, the score is “8”. • 8 points for NO • 0 for YES
