  3. IMPORTANT POINT TO REMEMBER The Fillable Application uses ALL the same information found in the Maryland Health Connections PAPER Application. You are capturing ALL the same information. THIS APPLICATION IS TEMPORARY – ONCE THE MHC SYSTEM IS AVAILABLE THIS APPLICATION WILL BE DEACTIVATED.

  4. Portal Access To access the on-line application Go to the DHR Intranet On the Right Hand Side is a list of Links Click on the link http://www.marylandhealthapp.com This takes you to the Application Log-In page
