Contd. … • Maintenance of Master Treatment Register and Individual patient treatment card. • Record of contact survey against Child & MB cases • [ • Availability of 2 months stock of MDT in hand against all cases under treatment. • Number of ASHA paid incentive on confirmation of Leprosy cases. • Number of ASHA paid incentive on completion of treatment.

  9. SUBCENTRE • ANM possess the treatment cards and MDT. • ANM identify new suspects and refer to PHC. • IPC during routine house visits – for Leprosy also. • Follow up cases under treatment and retrieve absentee in time.

  10. VILLAGE • Involvement and awareness of village leaders • Cause of Leprosy • Curability of disease • Mode of transmission • Participation of PAL in social functions • ASHA whether trained and involved in suspecting leprosy cases. • Whether ASHA paid incentive for any referred cases diagnosed as Leprosy and on completion of treatment.
