Some facts... • 75% of the over 75 population suffer from chronic disease, with incidences expected to double by 2030 • Demands on the healthcare system is increasing by 4% per year • There are 800,000 people with dementia in the UK with numbers set to rise to over 1 million by 2021. This will soar to 1.7 million by 2050

  6. What has PYF achieved so far? Approximately 2,800 people have had a PYF intervention • Out of Hours Transport • WRVS Befriending • Basic Footcare • Hard of Hearing Group • Living Well with Dementia • Community Resilience A&E • Building Healthy Communities

  7. What has PYF achieved so far? Many more people are receiving new or improved services as a result of PYF support • More people have the option of a teleclinic • Improved systems for care planning reviews • Improved use of mobile technology for community teams releasing staff time

What Has PYF Achieved so far? “It really is a win, win situation with this course – as a result, the Service Users receive more frequent simple nail care, our staff are further trained and developed and pressure on the stretched resources of the podiatry services are reduced” Staff Development
