9. Highly Reliable Performance Pantex “Flash Bang” incident demonstrates a superior focus on improving plant operations. Joint Labor and Management response drove operational security improvements through mutual decision-making, open communications, deference to expertise, and a systematic elimination of errors.

10. Highly Reliable Performance • Flash Sound Device Event A flash sound device prematurely detonated while being deployed during a Special Response team training exercise. The event was mitigated to a non injury event and reinforces the value of the ISM/ISSM program.

Highly Reliable Performance Individual Attitude and Responsibility Security Police Officers demonstrate a “Questioning Attitude” by challenging assumptions, investigating anomalies and considering the potential adverse consequences of planned actions. …ALL plant employees remain mindful of conditions or activities that can have an undesirable effect on safe performance

12. Highly Reliable Performance Video • The first portion of the video will identify the ISM/ISSM program structure as applied in a security environment. • The Special Response Team that participated in the recreated Flash Sound Device event, is the same team involved in the actual event.

Highly Reliable Performance Flash Bang incident presented an opportunity for Pantex Leadership – Management and Pantex Guards Union – to learn and improve security operations by using the ISM structure to assess existing operational risks and mitigate those risks to prevent future recurrence of similar incidents.

14. Highly Reliable Performance Security Police Officers A SPO’s safe performance is managed as a vital and valued part of the plant’s ISM program. SPO’s “own” their safety processes. Their input for improvement is valued and solicited and as a result, they work to continuously improve their safe performance.
