Core Benefits • Life Insurance & Accidental Death and Dismemberment, Long Term Disability (LTD) and Transition Assistance Program (TAP) • Eligible employees are automatically enrolled on the first of the month following 1 year of continuous benefit eligible employment • 1,000 hours per year • Premiums are 100% paid for by your location

Life Insurance and AD&D • Coverage amount is 2x base annual salary • Coverage amount reduces at age 65 and age 70 • AD&D – Double indemnity if you pass away in an accident, and benefits for dismemberment • Employee must designate a beneficiary through BAS/MyEnroll • www.bostoncatholicbenefits.org/life

  4. Long-Term Disability (LTD) Benefits are payable after 6 months of disability Monthly income benefit of 60% of base pay (maximum of $5,000 per month) Benefit payments are taxable www.bostoncatholicbenefits.org/ltd

Transition Assistance Program (TAP) Unemployment insurance administered by RCAB as a benefit paid to employees involuntarily terminated (except in cases of gross misconduct) 50% of base weekly wage, to a maximum adjusted annually to reflect the Massachusetts unemployment weekly benefit maximum Benefits are payable bi-weekly for a maximum duration of 30 calendar weeks Career Counseling Sessions available at no cost www.bostoncatholicbenefits.org/TAP
