Purpose This document is intended to help students with disabilities and families: • Navigate the transition process • Identify participants in the transition planning process • Identify participants’ responsibilities • Identify expectations of others based on your role in the IEP team • Identify possible resources

  4. Considerations • Expectations are broad expectations. • Your local school district may have additional expectations • Check in with your child’s school for additional expectations • Feel free to add local expectations within the notes section

  5. Acronyms to Consider: • IEP = individualized education program • IPE = individualized plan for employment • ISP = individualized service plan • PCP = person-centered plans • VR = vocational rehabilitation • DHS = Department of Human Services • SoP = Summary of Performance • DD = developmental disability
