  2. Each OKCPS K-4th student will be provided with an individualized Academic Progress Plan(APP). The plan will be created each year by the student’s primary reading teacher. This teacher will collaborate with the other members of the Academic Team concerning services for the student.

  3. The Academic Progress Plan(APP) will be discussed at the fall and spring parent-teacher conferences.

  4. The APP describes the child’s level of reading proficiency and the instructional supports and services provided by the school to help the child strengthen their skills in Reading.

  6. The 2014-2015 OKCPS APP is available to teachers in two ways: 1) A fillable PDF is available at http://tinyurl.com/okc-rsa. Teachers can type directly onto this document and then save/print their student’s documents. 2) Paper copies were provided to K-4 teachers at the BOY trainings. Please contact Amy Walls if you need additional copies.

  8. 1 1 Using information on the cumulative folder, indicate the number of times the student was previously retained in each grade. Page 1 of the APP

  9. Red High Risk Use BOY data for Fall, MOY for Winter, and EOY for Spring. Provide the color and the risk level. Page 1 of the APP

  10. Diagnostic Assessments Used as Necessary Include additional classroom Literacy Assessments in the green folder. In this box: 1) List the name of the assessment(s). 2) Indicate the date(s) the student was assessed. 3) In the ‘Results’ column either summarize the results or write “results included in folder.” Page 1 of the APP

  11. If you teach in a school-wide Title I school, mark the first box for every student. If not, the office will have a list of student’s who qualify. Mark the box for IDEA for any student who is on an IEP. (Service or monitor) Mark the third box for and student designated as ELL based on the ELL list provided by your building ELL coordinator. (Pull-out service or in-classroom support) Page 1 of the APP

  12. Page 2 of the APP informs parents of the legal requirements for promotion from third grade. These requirements will be reviewed at each K-3 conference. 4th grade teachers will briefly inform/remind parents that these requirements were met by their student last year in order to enroll in 4th grade this year.   14. A review of the Third-Grade Graduation Requirements is available here. Prior to conferences, make sure you are comfortable reading and discussing the possible promotion methods.

  15. The Beginning-of-Year Instructional Summary is completed for every APP that begins prior to Winter break. This section indicates all of the instructional services and supportsthat will be provided to the student during the first semester. Page 3 of the APP

  16. Every student receives 90 minutes with Treasures as the core program. Yellows and Reds receive Tier II. A specific program name is optional. Reds receive Tier III. A specific program name is optional. Every student receives PM. Indicate frequency. If tutoring* or Saturday School is provided to this student indicate the frequency. If this student is invited indicate the session(s). Check box if resources are provided at conferences.* Check box if login and password info is provided. Check box if student is involved in mentoring.* Check box & explain services for above-level students. Indicate services not listed above that are provided for this student by your school.   17. 3rd Grade ONLY *Students who were RSARetainedin 2013-2014 must be provided with: 1) Supplemental after-hourstutoring 2) “Read at Home” assistance plan OR 3) After-hours mentor or tutoring Third-Grade Teachers- indicate the option your school selected on the APP and explain to parents that support is provided in accordance with the law. Page 3 of the APP

  18. Use this section to indicate the 1-2 areas of strongest need for this student’s growth. Page 3 of the APP

  19. The Beginning-of-Year Home Connect is completed for every APP that begins prior to Winter break. This section indicates the areas for Parental involvement during the first semester. Page 3 of the APP

  20. Read with your child every night. Ask questions and have conversations as a family. Practice finding letters as you drive in the car. Tell jokes and practice rhyming. Time your child reading the same passage /book each day this week and watch their fluency improve. Use this section to list activities parents can do with their child to strengthen the areas most in need of growth. Page 3 of the APP

  22. At this point, indicate the student’s current easyCBM risk level. Reiterate the importance of catching up (or remaining strong) throughout this school year. Page 3 of the APP

  23. The signature section of the APP is: 1) an acknowledgement of the child’s current reading level and areas of need 2) notification to the parent of the Third-Grade Graduation Requirements 3) an acceptance of the indicated services and supports to meet the child’s needs. Page 3 of the APP

  24. Required In addition to the parent and teacher- an administrator or designee is required to attend conferences for all students whose risk level is red or yellow. Page 3 of the APP

  25. State law requires that within thirty days of the BOY easyCBM benchmark parents of K-4 students will be notified if their student is at risk for reading difficulties (red or yellow). Page 3 of the APP

  26. If the parent attends conferences but will not sign the APP: Simply write “Parent declined to sign at this time.” Please staplea the copy of the parent-teacher conference sign-in sheet as evidence that the meeting was held. Page 3 of the APP

  27. If the parent is unable to attend conferences: The parent will be sent the OKCPS Initial Notification Letter and the Third-Grade Graduation Requirements. These resources are available in the Parent Conference Section of: http://tinyurl.com/okc-rsa . Page 3 of the APP

  28. If the parent is unable to attend conferences: The notification letters must be sent: 1) By certified mail with the receipt stapled tothe signature section of the APP. 2) By email with a read-receipt stapled to the signature section of the APP. Page 3 of the APP

This section indicates all of the instructional services and supportsthat will be provided to the student during the second semester. For students who enroll in OKCPS after winter break the Beginning of Year Summary will not be completed. Page 4 of the APP

  43. EOY Conferences End-of-Year Conferences will be provided for all OKCPS 3rd grade families within the last 30 days of school. Fourth-grade students who received a conference-based “probationary promotion” will also be provided an End-of-Year Conference. Not included in the APP
