(continued) • Addendum/addenda will be issued with clarifications/changes • Proposals will be evaluated per the Submittal Requirements as stated in the RFP • All proposals are due PRIOR TO AND NO LATER THAN : • 3:00 PM, December 31, 2015 • Contract Term – July 1, 2016 – June 30, 2017 with four (4) one-year renewal options and optional 1-6 months if needed at the end of the option year. P&C 600 Form • Proposal due date and time • PCOs e-mail address • Offeror contact person and contact information • Acknowledge receipt of the RFP and insert the number of Addenda received • The completed P&C 600, becomes page one of the proposal August 31, 2015
Representations and Certifications • The completed R&C page, becomes page two of the proposal • Non-Profit Status Proof & subcontract requirements • Disabled Veterans Business Enterprise requirements • Certification of Debarment, Suspension, Current Cost or Pricing and Independent Pricing • Name, Title, Date & Signature August 31, 2015
Contract Conflict Certification • The completed Contract Conflict Certification Form, becomes page three of the proposal • Attach disclosures to certification form or declare “None” • Company/Organization • Name, Title, Date & Signature August 31, 2015
Indemnification Certification Form • The completed Indemnification Certification Form needs to be submitted with the proposal August 31, 2015
