  7. Accident 1 • Driver returning from an account with loaded high side • Driver took a drink of water • Started coughing uncontrollably • Blacked out and left the roadway • Struck several trees before stopping What could have been done differently?

  8. Accident 2 • Driver was parked on yard, released park brake before leaving • Didn’t reset park brake; but returned to office • Truck rolled across parking lot striking a parked trailer and other equipment • Stopped just short of damaging building What could have been done differently?

Accident 3 • Returning from an account, container loaded • Driver entered a curve with cruise control set • Noticed container leaning to left • Disengaged cruise control • Container fell off the left side of the truck • Spilling material across the road and an adjacent field What could have been done differently?
