ARCP • Annual Review of Competence Progression • All post MMC core & specialty trainees are required to undertake an annual assessment of their progress • CT1-ST7 • LTFT • Out-of-programme • Fixed term training contract e.g. LAT, MTI • No ARCP - Cannot progress to next training level • Some conceptual differences compared to the RITAs

The ARCP • Panel reviews evidence of competence contained in the trainees portfolio • Educational Supervisors ARCP structured report: ESSR • Meeting the trainee is not part of the assessment process • ARCP panels cannot assess competence, only look at the evidence • E-portfolio in ARCP in 2013 for CT1-ST6 trainees • Feedback generally positive from trainees!

  5. ARCP Outcomes (Programme) Satisfactory Progress 1. Achieving progress and competences at the expected rate Unsatisfactory or insufficient evidence (trainee must meet with panel) 2. Development of specific competences required – additional training time not required 3. Inadequate progress by the trainee – additional training time required 4. Released from training programme with or without specified competences 5. Incomplete evidence presented – additional training time may be required Recommendation for completion of training 6. Gained all required competences

ARCP 2014 • E-portfolio for all trainees • Timetable for 2014 - www.welshschool.co.uk • January 14th – Feb prog, call-backs, out-of-synch • June 3rd - ST7 & CT2 programme • June 19th – Regional: CT1, CT2 & LAT (fixed term appt), Cardiff • TBC – Regional: North Wales • July 1st - ST4, to include intro to higher training day • July 2nd - ST5 & STC • July 3rd – ST3 & ST6

ARCP preparation ‘Gold Guide’ states: • ‘The educational supervisor is the crucial link between the educational and workplace based appraisal process since the trainer’s report provides the summary of the assessment evidence for the annual review process. The outcome from the annual review underpins and provides evidence for the workplace based appraisal process which is designed to reassure employers that the performance of doctors in postgraduate training is satisfactory’ • ‘The trainee’s educational supervisor must ensure that the trainee: • Is aware of his/her responsibility to initiate workplace based assessments • Maintains an up-to-date log-book • Ensures that the trainee’s professional portfolio is adequately developed….”

ARCP preparation • “Trainees will not be ‘chased’ to provide the documentation by the required date, but should be aware that failure to do so will result in the panel failing to consider their progress” • Failure to produce evidence will inevitably lead to delay in training progression. • The paperwork is the trainee’s responsibility..but.. • Experience shows that a pro-active Educational Supervisor does help the trainee

Structured report • ESSR key document within e-portfolio • In addition, the College Tutor completes College Tutor training summary to include: • List of completed units of training • College tutor assessment based on: • Consultant group feedback from specialist modules; paeds, cardiac, neuro, ICM, 1 year of general training • Different emphasis to MSF, which will continue as normal • Welsh School standardised Consultant feedback form • Interim supervisors report • ES to complete at end of 6 month placement

  18. Unsatisfactory ARCP Outcome(Outcomes 2,3) • Advance notice must be given to the Welsh School if an unsatisfactory outcome is a possibility • Unsatisfactory WPB Assessment tools • Attitudes and behaviour concerns • Exam failure • Failed or incomplete units of training • FEEDBACK

CT2 vs CT3 trainees • CT3 year uniquely approved by GMC for Welsh School of Anaesthesia • Optional year; Medicine, EM, ICM with Anaesthesia • Apply early in CT2 year • Basic level curriculum MUST be completed in 2 years, all UoTs and Primary FRCA • If BLTC not achieved, 6 month ‘remedial’ training extension required • ACCS trainees – same 6 month ‘remedial’ training extension if BLTC not achieved

  24. National co-ordinated 2014 • Twice yearly NCR, Autumn & Spring • Run by West Midlands Deanery for RCoA • Entry levels: • CT1 Anaesthesia (2+optional 3rd year prog) • CT1 ACCS-Anaesthesia (3 year prog) • CT2 (1 year fixed-term post) – WALES only! • ST3 (5 year programme)

Eligibility criteria • CT1 • 18 months or less experience of anaesthetics by Aug 2014 • ACCS CT1 • 18 months or less experience of ACCS components by Aug 2014 • CT2 • At least 12 months experience/training in anaesthetics • ST3 • At least 21 months experience of anaesthetic training & 3 months ICM at CT/SHO level by Aug 2014 • Pass the Primary FRCA (London or Ireland) by time of interview

Interview format • 3 national standardised stations • Presentation station (10 minutes) • Structured clinical scenario (10 minutes) • Portfolio review (20 minutes) • 2 “independent” assessors • Questions drawn from National approved bank • Assessment of attributes / NTS • Global rating score - ‘gut feeling’ score!

ICM recruitment • National interviews, co-ordinated & run within WMD • ST3 anaesthesia appointees from August 2012 can only apply for dual CCT w ICM • Future proposal to end 18 month window rule & make upper limit at end of ST5 • If >18 months, possible to apply & seek CESR (CP) • 77/88 posts filled in August 2013 • 62% had an anaesthetics background & planning dual CCT • ICM aspiration to exceed 100 posts in August 2014

  37. Assessor training • Developed over last 12 months by RCoA • Not mandatory but strongly recommended • E-learning based training • All interviewer panel members will be given login • 1 hour = 1 CPD point
