Steps for Recruitment • Step 1: Identify & Recruit eligible community members or patients for lab screenings • Step 2: Conduct Lab Screenings • Step 3: Contact eligible participants to recruit for DPP curriculum class

Screening Planning • Make contacts • Nurses • Room availability • Schedule date • Post flyers, send emails, mail information • Order t-shirts
Screening Implementation • Station 1: Registration • Station 2: Nurses/Fingerstick • Station 3: Consultations • Station 4: T-shirt
Screening Followup • Step 1: Separate Community Worksheets – Determine Eligibility • Step 2: Add data to screening log • Step 3: Data Tech enters eligible pts into Master Registry • Step 4: Remind pts of lab screening or send letter for upcoming lab
