Funding Announcement Number: HHS–2008–IHS–EHC–0001. Catalog of Federal Domestic Assistance Numbers: 93.933. Key Dates: Letter of Intent Deadline: May 2, 2008. Application Deadline Date: June 20,2008. Review Date: July 21–August 1, 2008. Earliest Anticipated Start Date: September 1, 2008.

IHS Elder Care Long Term Care Grants – FY08 Category 1 – Assessment and Planning • What do our elders need and how do we provide it? • 2 Years • Up to 50K per year • Eligible service(s) • Sustainability Category 2 – Implementation • We know exactly what we want to do, we know it will work, we just need some help getting it started. • 2 years • Up to 75K per year • Eligible service(s) • Sustainability

  5. IHS Elder Care Long Term Care Grants – FY08 Category 1 - Assessment and Planning • Understanding the need for LTC • Specifics!!!! • Demographics!!!! • Leading to comprehensive plan or vision for LTC • Leading to a detailed business plan for a service or services

  6. IHS Elder Care Long Term Care Grants – FY 08 Category 2 – Implementation • Need clearly demonstrated • Fully developed plan • Including possibly a business plan • Clear path to sustainability after 2 years

IHS Elder Care Long Term Care Grants – FY 08 • If a long term care service or program is not eligible for contracting or compacting, it is not eligible for funding through the grant program. • Only long term care services that are primarily medical in nature are within the authority of the Indian Health Services • The programs or services must be designed to serve IHS beneficiaries.

IHS Elder Care Long Term Care Grants – FY08 Only medical long term care services are within the authority of the Indian Health Service A medical long term care service should be • based on an individual assessment by a qualified professional. • part of an individual care plan. • linked to a specific diagnosis or diagnoses.

IHS Elder Care Long Term Care Grants – FY 08 Examples of programs or services not eligible for grant funding: • Assisted Living • Custodial Nursing Home Care • Non medical transportation • Non medical nutrition • Socialization • Chore services • Social Elder Day • “PACE (Program of All-inclusive Care of the Elderly” as a specific CMS program

IHS Elder Care Long Term Care Grants Examples of programs or services that are potentially eligible for grant funding: • Personal care delivered in elder housing • Swing beds, skilled nursing services, medical rehabilitation services • Medical Transportation • Medical Nutrition Therapy • Activities in a therapeutic setting • Medically required safety modifications in the home • Medical model Elder Day Health • LTC programs integrating medical Long Term Care services using the “PACE” model.
