PCT plans should enable 90% roll out - 18% of their eligible cohort to be offered an NHS Health Check

5. Q3 Vital Signs Data - 2011/12
Expecting ~ 1.8 m NHS Health Checks to be offered at this stage:
~1.5 m offered in ’11-12 to date
~530,000 offered in Q3
~760,000 taken up in ’11-12 to date
~275,000 taken up in Q3
~50% take up rate in ’11-12
~52% take up rate in Q3

PCTs asked to:
ensure GPs are recording the data on patient records / programme complying with collection rules.
