The nurses’ assessment of older adults requires the ability to actively listen as well as to use all other senses to gather data. • An inexperienced nurse is often frustrated by the length of time needed for the geriatric assessment
- Encourage the client to focus on answering the questions. • - In an effort not to be rude(غير مهذب ),the nurse may allow the patient to continue providing unessential information. • Written forms and checklists can help the nurse to keep the client more focused.
Environment • Start with a room that is comfortably warm to the client. • The room should be adequately bright but with indirect lighting. • Straight-backed chairs with arms that are cushioned for comfort. • The examination table should be low and well padded(مبطنة(to protect from discomfort.
5- The head of the examination table should rise up, as some older adults may have difficulty lying flat for any amount of time. 6- Adequate space in the examination room to accommodate mobility aides. 7- The room should be free from noises.
Various Techniques Use At Start Of Examination: • As a sign of respect, older adults should be addressed by their last name and title. • The nurse should speak facing the client. • use commonly accepted wording. • Allowing hearing. (If the client wears hearing aids, make sure they are on and working properly).
5. For clients with visual deficits, nurses must make sure that the clients have their glasses on and plan to use visual cues as needed. • Note: Family members can provide important information, but the examiner needs to focus on the client.
Geriatric interdisciplinary teams (GITs) It made up of : • physicians , nurses, physical therapists, occupational therapists, • recreational (ترفيهي)therapists, social workers, psychologists, and nursing assistants.
(GITs) cont……… • Geriatric interdisciplinary team care has been effective: • in managing the complex syndromes experienced by chronically ill and frail older adults with multiple co-morbidities, because such care requires skills that are not possessed by any one professional .
How the older adult would like to be addressed and the language: - That they are most comfortable speaking. If the older adult speaks a language foreign to the nurse, the client should be questioned as to whether or not an interpreter is desired or whether a family member would like to communicate the client’s history.
Attention should also be paid to the older adult’s comfort with: • The amount of personal space, • Eye contact, • Physical gestures of the health care provider.
The relationship of the nurse to the client requires recognition of and sensitivity to cultural differences, because some cultural groups definitions of health and illness may differ from the examiners.
When conducting assessments on older adults, it is also necessary: To remember that some of the standardized assessment tools, such as the Geriatric Depression Scale and the Mini Mental State Examination, are available in different languages. Be cautious about interpreting a tool.
