1. Network Mapping We are collecting information to draw a beginning map of our network. We know from research about change that understanding our network helps us build effective relationships and solve tough problems. Network maps have been powerful tools in other collaboratives such as the Pittsburgh VA and Regenstreif Institute work on MRSA. So, please take just a few minutes to fill out our questionnaire.

PLEASE DO Each person should do the following • Make sure your name is on the list – if not add it. • Fill out the first page by answering the questions • For each name on the list: • In the first column, put a check mark next to the names of people that you HAVE worked with in the past on infection control • In the next column, put a check mark next to the names of people that you WANT to work with on infection control • A TIP: Start with your facility, then your cluster, and go from there. Return your form to us today!
