  1. Commissioning plan 2014/19

  2. The NHS belongs to the people: a call to action • Healthcare is changing • Health is changing too • Services need to change too • We must achieve the best outcomes for our patients within the available budget.

  3. Services need to be organised on a locality basis, matching CCG and local authority boundaries • We need to review the type of services that are available within a community setting and those that are delivered in hospital.  http://www.england.nhs.uk/wp-content/uploads/2013/07/nhs_belongs.pdf

  4. Northumberland’s work is split into six clinical areas

  5. How we built our plan

  6. We have three key areas

  7. What has our CCG done so far?

  8. Good News Significant reduction in emergency admissions since the intro of the ambulatory care scheme and high risk patient programme. A number of admissions are avoided entirely whilst other patients are treated within the outpatient setting instead and then discharged on the same day

  9. Good News The Northumberland (local) target is 71% of Category A (red) incidents responded to within 8 minutes

  10. The initiatives 2014 – 2017

  11. Engagement • With patients and the public, members, providers and other key stakeholders • Face to Face • Online • Questionnaires • In partnership (alongside other events)
