  6. Applying for E-rate • Obtain a registration number from the FCC (FCC RN):https://fjallfoss.fcc.gov/coresWeb/publicHome.do • Apply online Form 470 and 471:http://www.sl.universalservice.org/menu.asp

Ordering Services – Form 471 The “Who I Am and the Services/Products I Am Ordering” Form • File within the window- Deadline March 24th 11:59 p.m. EDT • File ONLINE • File two 471’s; One for Priority 1 and one for Priority 2! • Take care to specify the correct category of service:Telecommunications or Internet access • Ensure the service requested is listed on the Form 470 you cite • Select appropriate originating Form 470 for multiyear contracts • Be able to justify amount of funding requested–have bills/contract ready to submit

  24. Next Steps • Complete Item 21 Attachments • Online • Paper • Certify Form 471 • Online with PIN • Print, sign, date and mail to the SLD at the address on the bottom of the certifications

Application Review Program Integrity Assurance (PIA), USAC reviews your Form(s) 471 to: • Check the eligibility of the schools and libraries and their discount levels • Verify that the services you requested are eligible for discounts • Receive a Receipt Acknowledgement Letter (RAL) • Make corrections to RAL if necessary (20 Days) Receipt Acknowledgement Letter (RAL)

Funding Commitments Receive your Funding Commitment • Following application review, USAC issues a Funding Commitment Decision Letter (FCDL) to both the applicant and the service provider(s) • Carefully review the FCDL for details on approved or denied requests and NEXT STEPS • Complete Form 486 • Must be filed within 120 days of the start of service or Funding Commitment Letter- whichever is LATER • If deadline is missed: Funding will be reduced one day for each day the Form is late!
