Lessons learnt from accident 25.10.2005 • CERN side • Need of clear separation of responsibilities between CERN and contractor • Respect official communication channel with the contractor (foremen) • Respect of administrative & safety procedures (AOC, VIC, EDH requests, etc.) • Mobbing accusation

  4. Lessons learnt from accident 25.10.2005 • Contractor side • Need to closely supervise his teams  rigorous communication channel • Whole responsibility of the operation, including risk assessment and operating method • Follow procedures in place

  6. Preparation, planning & Organisation • Clear definition of the activity to be performed • Overview of the activities in progress • Preparation of the material • Priorities and execution time

  7. Practical Organisation • Automatic EDH from MTF • Follow-up of weekly meetings: • One generic weekly EDH for basic operations • Individual EDH for planned operations (date range to cover planning changes) • Possibility to use services from TS-IC-LO staffs to create the EDH
