Referral Pathway – Counselling for Carers

1. Referral from professional (with carer’s consent) by online referral form or ‘phone contact with Carers Centre Adult Carers Team Cover Worker Adult Carers Team Cover Worker (normally available by ‘phone Mon-Fri 9-12 and 1-4) check s if carer already known to Carers Centre Cover Worker contacts carer to discuss options of Rock and Relate and refers if appropriate Referral Pathway – Counselling for Carers If inappropriate referral or service not available Cover Worker advises carer of alternative routes, e.g. G.P. or other counselling options Rock Clinic/Relate make contact directly with the carer to arrange initial assessment Cover Worker gives information to carer about other Carers Centre services and adds to relevant mailing lists if required, e.g. Carers News Allocation to Support and Outreach Worker by ACT Manager once space available and depending on priority Casework prioritised by Adult Carers Team Manager If 1:1 casework is required Cover Worker adds carer to casework waiting list
